摘　　要：目的 探讨不同消毒方法对控制非住院血透患者中心静脉置管相关感染发生率。方法 选择非住院的中心静脉置管血透患者104人为研究对象,随机分为碘伏组和氯己定醇组,对两组消毒维护后的消毒效果、敷贴粘贴牢固情况及导管相关感染情况等进行比较。结果 两组患者在维护前及维护后的细菌数量及消毒合格率等均无显著差异(p>0.05)。两组患者的消毒液干燥时间及消毒维护时间均有显著差异(p<0.05)。两组患者的敷贴起翘或脱落、导管相关感染及血透导管拔管情况均有显著差异(p<0.05)。结论 碘伏和氯己定醇用于非住院中心静脉置管血透患者的消毒维护,都能取得较好的消毒效果,而使用氯己定醇可以节约时间成本,敷贴起翘或脱落、导管相关感染及导管拔管率均较低。Objective To explore the effect of different disinfection methods on controlling the incidence of infection associated with central venous catheterization in hemodialysis patients. Methods One hundred and four hemodialysis patients who were not hospitalized with central venous catheter were randomly divided into iodophor group and chlorhexidine group. The disinfection effect, sticking firmness of the application and catheter-related infection after disinfection and maintenance were compared between the two groups. Results There was no significant difference in the number of bacteria and the qualified rate of disinfection between the two groups before and after maintenance(p0.05). There were significant differences in disinfectant drying time and disinfection maintenance time between the two groups(p0.05). There were significant differences between the two groups in sticking up or falling off the application, catheter-related infection and extubation of hemodialysis catheter(p0.05). Conclusion Both iodophor and chlorhexidine alcohol could achieve good disinfection effect when used for disinfection and maintenance of hemodialysis patients with indwelling catheter in central hospital. Using chlorhexidine alcohol could save time and cost, and the sticking or falling off of application, catheter-related infection and catheter extubation rate were all low.
