摘　　要：目的探讨基于加速术后康复(ERAS)的临床护理路径联合情志护理对接受经导管动脉化疗栓塞(TACE)的肝细胞癌患者并发症、疼痛、睡眠和生活质量的影响。方法选取2018年1月—2021年1月在医院接受TACE的肝细胞癌患者370例,按照组间基本特征匹配的原则分为对照组和观察组,各185例,对照组实施常规护理干预,观察组在对照组基础上实施ERAS的临床护理路径联合情志护理。比较两组并发症发生率、疼痛、睡眠状态和生活质量评分。结果观察组并发症总发生率(14.59%)低于对照组(33.51%),差异具有统计学意义(P<0.05)。观察组VAS评分低于对照组,差异具有统计学意义(P<0.05)。观察组睡眠状态评分低于对照组,差异具有统计学意义(P<0.05)。观察组生活质量评分高于对照组,差异具有统计学意义(P<0.05)。结论基于ERAS的临床护理路径联合情志护理可有效减少肝细胞癌TACE患者的并发症;减轻疼痛;改善睡眠状态;提高生活质量。Objective To explore the effect of enhanced recovery after surgery(ERAS)based clinical nursing pathway combined with emotional care on complications,pain,sleep,and quality of life in hepatocellular carcinoma patients receiving transcatheter hepatic arterial chemoembolization(TACE).Methods A total of 370 hepatocellular carcinoma patients who underwent TACE in hospitals from January 2018 to January 2021 were selected and divided into a control group and an observation group based on the principle of matching basic features between groups,with 185 cases in each group.The control group received routine nursing intervention,while the observation group received ERAS clinical nursing pathway combined with emotional care on the basis of the control group to compare the incidence of complications,pain,sleep status,and quality of life scores between the two groups.Results The total incidence of complications in the observation group(14.59%)was lower than that in the control group(33.51%),the difference was statistically significant(P<0.05).The VAS score in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The score of sleep status in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The quality of life score in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion The combination of ERAS based clinical nursing pathway and emotional care can effectively reduce the complications of TACE patients with hepatocellular carcinoma,reduce pain;Improve sleep status,and improve the quality of life.
