摘　　要：目的:制定华法林在非瓣膜性房颤中抗凝治疗的标准方案及处方审核细则。方法:基于医院信息系统、房颤诊疗指南、华法林说明书、临床路径和专业文献,并结合专家调研意见及临床用药经验,从适应证、用法用量、特殊人群、相互作用、监护要点、出血事件等16个方面梳理华法林处方审核细则,在临床药学实践中加以修订。结果:华法林处方审核细则信息全面、便捷高效,明显提高了处方审核的权威性及规范性。结论:华法林处方审核细则可为药师开展处方审核工作提供借鉴。OBJECTIVE To formulate the standard prescription review rules of warfarin anticoagulation therapy in non-valvular atrial fibrillation.METHODS Based on the hospital information system,atrial fibrillation diagnosis and treatment guidelines,warfarin instructions,clinical pathways and professional literature,combined with expert research opinions and clinical medication experience,the review rules of warfarin prescriptions were summarized from16 aspects,including indications,usage and dosage,special population,interaction,monitoring points,bleeding events,and revised in clinical pharmacy practice.RESULTS The warfarin prescription audit rules were comprehensive and easy to apply,which improved the efficiency and standardization of the prescription review obviously.CONCLUSION The warfarin prescription review rules embody the concept of medication treatment management and provide reference for carrying out prescription review and pharmaceutical supervision.
