慢性鼻窦炎术后局部激素不同给药方式研究进展

摘　　要：作为耳鼻喉科多发性高度异质性疾病,慢性鼻窦炎具有非常复杂的发病机制,诱发因素包括环境因素、遗传因素及解剖结构等,对患者正常生活可造成较大的干扰,尤其是伴嗜酸性粒细胞浸润及鼻息肉的慢性鼻窦炎具有治疗难度大以及预后差等特点,对患者的身心健康可产生极大的不良影响。临床普遍采用包括手术在内的综合治疗,术后配合规范有效的治疗手段。但是患者术后复发率及再手术率均较高。手术为临床治疗慢性鼻窦炎的重要手段,并非改变鼻窦黏膜炎症性质,而是为术后局部用药尤其是鼻用激素提供渠道。鼻用糖皮质激素属于慢性鼻窦炎术后一线用药,疗效确切,对于促进患者健康恢复能够发挥积极作用。近年来,专家学者针对慢性鼻窦炎术后局部激素的给药方式开展了大量的研究,包括鼻腔雾化吸入、鼻腔灌洗、鼻滴剂以及鼻喷剂、鼻腔激素植入物等。本文就不同给药方式进行综述,以期为提高临床疗效提供参考。As a multiple and highly heterogeneous disease in the otorhinolaryngology department,chronic nasosinusitis has a very complex pathogenesis.The predisposing factors include environmental factors,genetic factors and anatomical structure,etc.,which can cause great interference to the normal life of patients,especially chronic nasosinusitis with eosinophilic infiltration and nasal polyps has the characteristics of difficult treatment and poor prognosis,which can have great adverse effects on the physical and mental health of patients.Comprehensive treatment methods including surgery are generally used in clinical practice,and standardized and effective treatment methods are used after surgery.However,the postoperative recurrence rate and reoperation rate are high.Surgery is an important means of clinical treatment of chronic nasosinusitis,not to change the inflammatory nature of the sinus mucosa,but to provide a channel for postoperative topical medication,especially nasal hormones.Nasal corticosteroids belong to the first-line drugs after chronic nasosinusitis surgery,with exact curative effect,and can play a positive role in promoting the recovery of patients'health.In recent years,experts and scholars have carried out a lot of research on the administration of topical hormones after chronic nasosinusitis surgery,including nasal aerosol inhalation,nasal lavage,nasal drops,nasal sprays,and nasal hormone implants.The paper will review different administration methods,in order to provide reference for improving clinical efficacy.
