蒲黄消瘿贴治疗桥本甲状腺炎伴结节性甲状腺肿临床观察

摘　　要：目的观察蒲黄消瘿贴治疗桥本甲状腺炎(HT)伴结节性甲状腺肿的临床效果。方法采用随机双盲研究,将90例HT伴结节性甲状腺肿患者均分为观察组(蒲黄消瘿贴)和对照组(安慰剂贴),比较干预12周前后甲状腺功能[游离三碘甲腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))、超敏促甲状腺激素(sTSH)]、甲状腺自身抗体[甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TgAb)]、甲状腺大小及甲状腺结节大小变化。结果2组治疗前后FT_(3)、FT_(4)、sTSH差异均无统计学意义(P>0.05);观察组TPOAb、TGAb较治疗前明显降低(P<0.01),对照组治疗前后差异无统计学意义(P>0.05),观察组治疗后TPOAb、TGAb低于对照组(P<0.05,P<0.01);观察组治疗后甲状腺结节大小、个数小于对照组(P<0.05,);观察组治疗总有效率77.78%(35/45)高于对照组的20.00%(9/45)(P<0.01)。结论蒲黄消瘿贴可缩小HT患者结节性甲状腺肿,降低甲状腺自身抗体。Objective To observe the clinical effect of Puhuang Xiaoying paste in the treatment of Hashimoto’s thyroiditis(HT)with nodular goiter.Methods In a randomized double-blind study,90 patients with HT with nodular goiter were divided into observation group(Puhuang Xiaoying paste)and control group(placebo paste).The changes of thyroid function(FT_(3),FT_(4),sTSH),thyroid autoantibodies(TPOAb,TGAb),thyroid size and thyroid nodule size before and after the intervention for 12 weeks was compared.Results There was no statistical significance in FT_(3),FT_(4) and sTSH between the two groups before and after treatment(P>0.05),TPOAb and TGAb in the observation group were significantly reduced compared with those before and after treatment(P<0.01),and there was no statistical significance in the control group before and after treatment(P>0.05).TGAb was lower than that of control group(P<0.05,P<0.01).The size and number of thyroid nodules in observation group were smaller than those in control group after treatment(P<0.05,).The total effective rate of the observation group was 77.78%(35/45),which was higher than that of the control group(20.00%,9/45)(P<0.01).Conclusion Puhuang Xiaoying paste can reduce nodular goiter and thyroid autoantibodies in patients with Hashimoto’s thyroiditis.
