颈椎操联合针刺治疗神经根型颈椎病临床观察

摘　　要：目的观察针刺配合颈椎操对神经根型颈椎病患者的临床症状及血浆P物质(SP)及肿瘤坏死因子-α(TNF-α)的影响。方法将58例神经根型颈椎病患者按随机数字表法分为治疗组(29例)、对照组(29例)。治疗组患者采用针刺配合颈椎操治疗,对照组患者采用针刺治疗。分别于治疗前、治疗12 d后评估2组患者的临床症状改善情况,测定2组患者的血浆SP及TNF-α浓度。结果在治疗12 d后,2组患者总有效率差异无统计学意义(P>0.05),愈显率治疗组明显高于对照组(P<0.05)。组内比较,2组患者治疗后血浆SP浓度及TNF-α浓度均较治疗前明显降低(P<0.001);组间比较,治疗组较对照组降低更加明显,差异有统计学意义(P<0.001)。结论针刺配合颈椎操可有效抑制神经根型颈椎病患者SP及TNF-α的释放,提高临床疗效。Objective To observe the effects of acupuncture combined with cervical vertebra exercises on plasma SP(substance P)and TNF-α(plasma tumor necrosis factor)in patients with cervical spondylotic radiculopathy.Methods Fifty-eight patients with cervical spondylotic radiculopathy were randomly divided into a treatment group and a control group.29 patients in the treatment group were treated with acupuncture combined with cervical spine exercises,and 29 patients in the control group were treated with acupuncture.The plasma SP and TNF-αconcentrations of each group of patients were measured before and after treatment.Results After the two groups of patients were treated for 12 days,the clinical efficacy was compared,the total effective rate was not statistically significant between the two groups(P>0.05),and the recovery rate of the treatment group was significantly higher than that of the control group(P<0.05).Plasma SP concentration and TNF-αconcentration after treatment in the two groups were significantly lower than before treatment(P<0.001),and the difference between the treatment group and the control group was statistically significant(P<0.001).Conclusion Acupuncture combined with cervical vertebra exercises can effectively inhibit the release of SP and TNF-αand improve clinical effect.
