摘　　要：目的探讨自制顺行髓内针内固定治疗第5掌骨颈骨折的方法及疗效。方法2016年3月-2021年3月,对15例明显移位的第5掌骨颈骨折患者利用自制顺行髓内针内固定方法给予治疗。术后无需石膏托固定,早期活动。结果15例患者术中均获得解剖复位,切口均一期愈合。患者均得到随访,随访时间6~8个月,髓内针6~7周拔除,骨折均在8~12周完全骨性愈合。其中14例解剖愈合,1例存在15°掌侧成角畸形愈合。术后3个月患侧掌指关节活动度为85.35°±3.31°,与健侧87.25°±2.17°比较,差异无统计学意义(P>0.05)。结论采用自制顺行髓内针经髓撬拨复位的方法固定第5掌骨颈骨折,手术创伤小,操作简便,术后并发症少,关节功能恢复良好。Objective To explore the method of self-made antegrade intramedullary nail internal fixation in the treatment of the fifth metacarpal neck fracture.Methods From March 2016 to March 2021,15 patients with significantly displaced the fifth metacarpal neck fractures were treated with self-made antegrade intramedullary nail internal fixation.There was no need for plaster support fixation and early movement after operation.Results Anatomical reduction was achieved in all 15 patients,and the incisions healed in one stage.All patients were followed up for 6-8 months.Intramedullary nails were removed in 6-7 weeks,and fractures healed completely in 8-12 weeks.Among them,14 patients had anatomical healing,and 1 patient had 15°volar angulation malunion.The range of motion of the metacarpophalangeal joint on the affected side was 85.35°±3.31°,which was not statistically significant compared with the healthy side 87.25°±2.17°(P>0.05).Conclusion Using self-made antegrade intramedullary nail to fix the fracture of the fifth metacarpal neck through medullary prying reduction has the advantages of less trauma,simple operation,less postoperative complications and good recovery of joint function.
