摘　　要：目的探讨克氏针内固定与钢板内固定治疗第5掌骨骨折的疗效。方法回顾性分析2018年8月-2021年8月收治的第5掌骨骨折患者82例,随机分为克氏针组(41例,克氏针内固定)、钢板组(41例,钢板内固定)。术后评估患手功能恢复情况,记录围术期指标,测量手术应激指标,统计术后并发症。结果克氏针组手部功能恢复优良率高于钢板组(P<0.05);克氏针组术中失血量、手术时长、住院时间均低于钢板组(P<0.05);术后1 d克氏针组血清去甲肾上腺素(NE)、皮质醇(Cor)水平显著低于钢板组(P<0.05);克氏针组并发症发生率显著低于钢板组(P<0.05)。结论第5掌骨骨折治疗中克氏针内固定较钢板内固定创伤性更小,患者应激反应更轻,术后并发症更少,康复更快。Objective To explore the difference in efficacy between Kirschner wire internal fixation and plate internal fixation in the treatment of fifth metacarpal fractures.Methods A total of 82 patients with fractures of the fifth metacarpal bone treated in our hospital from August 2018 to August 2021 were included and randomly divided into a Kirschner wire group(41 cases,internal fixation with Kirschner wire)and a steel plate group(41 cases,internal fixation with a steel plate).Fixation,evaluate the patient's hand function recovery,record perioperative indicators,measure surgical stress indicators,and count postoperative complications.Results The excellent and good rate of hand function recovery in the Kirschner wire group was higher than that in the steel plate group(P<0.05).Serum levels of norepinephrine(NE)and cortisol(Cor)in the Kirschner wire group were significantly lower than those in the steel plate group(P<0.05);the incidence of complications in the Kirschner wire group was significantly lower than that in the steel plate group(P<0.05).Conclusion Kirschner wire fixation is less invasive than steel plate fixation in the treatment of fifth metacarpal fractures,the patients have less stress response,fewer postoperative complications and faster recovery.
