药学监护路径在非内分泌专科糖尿病住院患者中的应用与效果

摘　　要：目的探索临床药师在非内分泌专科糖尿病住院患者中开展药学监护的方法和工作模式,以规范降糖药物的合理用药。方法应用临床路径的基本原理和方法,结合临床药师的工作实际,围绕降糖药物治疗的全过程,构建以患者病情评估、用药审查、疗效评价、药学干预、用药教育及不良反应监护等为内容的非内分泌专科住院患者的糖尿病药学监护路径。结果以神经内科为例,实施糖尿病药学监护路径后,患者出院前血糖达标率、平均最大血糖波动幅度、用药不合理率、药师干预率及药物不良反应发生率等各项指标均较实施前显著改善。结论临床药师在非内分泌专科住院患者中实施糖尿病药学监护路径,有利于提高降糖疗效、促进合理用药、保障用药安全。Objective To explore ways in which clinical pharmacists give pharmaceutical care among inpatients with diabetes mellitus in departments other than the department of endocrinology,and to standardize hypoglycemic treatment.Methods By adopting the principles and methods of clinical pathways in the whole process of hypoglycemic treatment and by using their expertise,clinical pharmacists constructed a pharmaceutical care pathway for inpatients with diabetes mellitus in departments other than the department of endocrinology,involving disease assessment,review of medication,efficacy evaluation,pharmaceutical intervention,medication training,and adverse reaction monitoring.Results After the implementation of the diabetic pharmaceutical care pathway,the patients'pre-discharge blood glucose compliance rate,average largest amplitude of glycemic excursions,irrational drug usage,the rate of pharmacist intervention and the rate of adverse drug reactions were significantly improved.Conclusion The diabetic pharmaceutical care pathway implemented by clinical pharmacists among non-endocrinology department inpatients can help improve the hypoglycemic effect,promote rational medication,and ensure the safety of medication.
