摘　　要：目的通过对老年患者高血压药物治疗管理,探讨钙离子拮抗剂、利尿剂间产生处方级联的关系。方法临床药师参与1例高血压老年疗养患者的药物治疗管理,识别并中断氨氯地平与螺内酯间的处方级联,减轻级联药物对患者身体损害,为临床提供参考。结果疗养患者更换不同厂家的氨氯地平及停用螺内酯的治疗方案,双下肢水肿及皮肤过敏症状基本消失。结论老年高血压患者为处方级联高危人群,药师应加强药学服务,识别处方级联,优化处方,保障患者用药安全。Objective To explore the relationship between calcium antagonists and diuretics in elderly convalescent patients with hypertension.Methods Clinical pharmacists participated in the pharmaceutical care of an elderly patient with hypertension,identified and discontinued the prescribing cascades between amlodipine and spironolactone,and reduced the damage to the patient's body.Results Convalescent patients used amlodipine from other manufacturers and discontinued spironolactone.The edema of lower limbs and skin allergy disappeared.Conclusion Elderly hypertensive patients are at high risk of prescribing cascades.Pharmacists should strengthen pharmaceutical care,identify prescribing cascades,optimize prescriptions,and ensure medication safety.
