摘 要:目的:对比分析无变形弥散加权成像(ARMS DWI)与常规多次激发脉冲平面回波(MS⁃EPI)弥散加权成像(DWI)技术对急性脑梗死图像质量的影响。方法:收集2021年5月至2022年3月在本院接收诊治的急性脑梗死患者40例,采用磁共振进行ARMS DWI和MS⁃EPI DWI序列扫描。分别计数2组图像中脑梗死病灶数量和位置;对2种序列的磁敏感伪影、病灶边缘锐利度、脑实质解剖结构显示度和图像整体质量进行评分;并进行客观评估,包括测量脑梗死病灶的信噪比(SNR)、对比度(CR)及表观弥散系数(ADC)。对采集的各种数据进行统计学分析。结果:40例患者共检出急性脑梗死病灶133处,其中MS⁃EPI DWI序列正确检出130处(另有12处识别为急性脑梗死,后被证实为磁敏感伪影),未检出3处,而ARMS DWI序列都正确检出所有病灶。MS⁃EPI DWI序列诊断急性脑梗死的灵敏度、特异度、准确度分别为97.7%、92.9%、95.0%;而ARMS DWI序列诊断的灵敏度、特异度、准确度均为100%。ARMS DWI显示磁敏感伪影、病灶边缘锐利度、脑实质解剖结构显示度和图像质量评分的分数都高于MS⁃EPI DWI(均P<0.05)。ARMS DWI中的SNR低于MS⁃EPI DWI(P<0.05),但CR高于MS⁃EPI DWI(P<0.05);ARMS DWI序列的脑梗死ADC值高于MS⁃EPI DWI序列(P<0.05)。结论:ARMS DWI图像无磁敏感伪影,特别在颅底岩骨周围、颞叶、副鼻窦近额叶等处可以提高急性脑梗死的诊断准确度。Purpose:To analyze the effect of acquisition and reconstruction with motion suppression diffusion weighted imaging(ARMS DWI)and conventional multiple shot echo planar imaging(MS-EPI)DWI on the image quality of acute cerebral infarction using 3.0 T magnetic resonance.Methods:Forty patients with acute cerebral infarction were collected and undergone ARMS DWI and MS-EPI DWI sequence scans from May 2021 to March 2022.The number and the position of cerebral infarctions were analyzed.The magnetic susceptibility artifacts,sharpness of cerebral infarction,brain parenchyma anatomical structure and the overall image quality were scored.The image quality was also evaluated objectively.The evaluation indexes included the signal-to-noise ratio(SNR),contrast ratio(CR)and apparent diffusion coefficient(ADC).All collected data were statistically analyzed.Results:A total of 133 lesions of acute cerebral infarction were detected in 40 patients,among which 130 lesions were correctly detected in the MS-EPI DWI sequence(another 12 detected lesions of acute cerebral infarction were confirmed as magnetic sensitive artifacts later),and 3 lesions were not detected,while all lesions were correctly detected in the ARMS DWI sequence.The sensitivity of MS-EPI DWI sequence for the diagnosis of acute cerebral infarction was 97.7%,the specificity was 92.9%,and the accuracy was 95.0%.The sensitivity of ARMS DWI sequence for the diagnosis of acute cerebral infarction was 100%,the specificity was 100%,and the accuracy was 100%.ARMS DWI showed higher image quality scores than MS-EPI DWI(P<0.05).The SNR in ARMS DWI was lower than that in MS-EPI DWI(P<0.05),but CR was higher than that in MS-EPI DWI(P<0.05).The ADC value of cerebral infarction in ARMS DWI was higher than that in MS-EPI DWI(P<0.05).Conclusion:There are no magnetic susceptibility artifacts in ARMS DWI images,which can improve the diagnostic accuracy of acute cerebral infarction,especially in the area around the petrous bone of skull base,temporal lobe and paranasal sinus near frontal lo
