摘  要:【目的】探讨促醒针法对颅脑损伤昏迷大鼠的促醒作用及其可能机制。【方法】将SD大鼠随机分为假手术组、模型组、促醒针法组,每组8只。模型组和促醒针法组大鼠应用脑立体定位精密打击仪制作颅脑损伤昏迷模型。促醒针法组施予针刺。治疗后,评估大鼠觉醒程度,采用苏木素-伊红(HE)染色法观察大鼠脑顶叶组织的病理变化,并计算正常神经元数目,Western Blot法检测大鼠额叶、顶叶和下丘脑组织的食欲素A(Orexin-A)、食欲素受体1(OX1R)表达。【结果】与假手术组比较,模型组大鼠意识状态评分降低(P<0.05),脑顶叶损伤区组织可见水肿、神经元细胞损伤,正常神经元数目减少(P<0.05),额叶、顶叶和下丘脑Orexin-A及OX1R蛋白表达水平降低(P<0.05);与模型组比较,促醒针法组大鼠意识状态评分升高(P<0.05),脑顶叶损伤区组织病理改变明显改善,正常神经元数目增多(P<0.05),额叶、顶叶和下丘脑Orexin-A及OX1R蛋白表达水平升高(P<0.05)。【结论】促醒针法可有效促醒颅脑损伤后昏迷大鼠,其机制可能与上调额叶、顶叶及下丘脑部位Orexin-A、OX1R表达,减轻神经元损伤有关。Objective To explore the effect and its possible mechanism of awakening acupuncture on comatose rats with traumatic brain injury(TBI).Methods The SD rats were randomly divided into the shamoperation group,model group and awakening acupuncture group,with 8 rats in each group.The rats in the model group and the awakening acupuncture group were subjected to a TBI comatose model using a brain stereotactic precision percussion instrument.The awakening acupuncture group was given acupuncture.After treatment,the arousal level of rats was evaluated.The histopathological changes in the parietal lobe were observed by hematoxylin-eosin(HE)staining,and the number of normal neurons was calculated.The expressions of Orexin-A and Orexin receptor 1(OX1R)in the frontal lobe,parietal lobe and hypothalamus were detected by Western Blot.Results Compared with the shamoperation group,the scores of conscious state of rats in the model group were decreased(P<0.05),edema and neuronal cell injuries were seen in damaged tissue of parietal lobe,and the number of normal neurons was decreased(P<0.05),protein expression levels of Orexin-A and OX1R in frontal lobe,parietal lobe and hypothalamus were decreased(P<0.05).Compared with the model group,conscious state scores were increased(P<0.05),histopathologic changes in damaged area of parietal lobe were significantly improved,and the number of normal neurons was increased(P<0.05),and protein expression levels of Orexin-A and OX1R in frontal lobe,parietal lobe and hypothalamus were increased(P<0.05).Conclusion Awakening acupuncture can effectively induce wakefulness in comatose rats with TBI,and its mechanism may be related to the upregulation of expressions of Orexin-A and OX1R in the frontal lobe,parietal lobe and hypothalamus,and the reduction of neuronal injury.
