复方黄连液湿敷促进肩峰成形术后早期功能康复的临床研究

摘　　要：【目的】观察复方黄连液湿敷对肩关节镜下肩峰成形术治疗肩峰撞击综合征患者术后早期功能康复的影响。【方法】将50例拟行手术治疗的肩峰撞击综合征患者随机分成观察组和对照组,每组各25例。2组患者均给予肩关节镜下肩峰成形术治疗,观察组在此基础上予以复方黄连液外敷治疗,每日2次,疗程为2周,于术后4周评价疗效,并观察2组患者术前和术后2周、术后4周的肩关节疼痛视觉模拟量表(VAS)评分及美国加州大学肩关节评分量表(UCLA)评分的变化情况。【结果】(1)疗效方面:术后4周,观察组的总有效率为92.00%(23/25),对照组为80.00%(20/25);组间比较(秩和检验),观察组的疗效明显优于对照组(P<0.05)。(2)疼痛程度评估方面:术后2周和4周,2组患者的VAS评分均较术前明显降低(P<0.05),且观察组在术后2周和4周对VAS评分的降低作用均明显优于对照组(P<0.05)。(3)肩关节功能评估方面:术后2周和4周,2组患者的UCLA评分均较术前明显升高(P<0.05),且观察组在术后2周和4周对UCLA评分的升高作用均明显优于对照组(P<0.05)。【结论】肩峰成形术后辅以复方黄连液湿敷,能加速肩关节肿胀的消退,有效缓解术后疼痛症状,改善患者的肩关节功能。Objective To observe the effect of compound Huanglian Lotion wet compress on the early functional rehabilitation of patients with subacromial impingement syndrome treated by arthroscopic acromioplasty.Methods Fifty patients with subacromial impingement syndrome to be going to take the surgery were randomly divided into observation group and ontrol group,with 25 patients in each group.Patients in both groups were treated with arthroscopic acromioplasty,and additionally the observation group was treated with topical application of compound Huanglian Lotion twice per day for 2 weeks.The therapeutic effect was evaluated 4 weeks after the surgery,and the changes in the visual analogue scale(VAS)scores of shoulder pain and the scores of University of California at Los Angeles shoulder function rating scale(UCLA)were observed before the surgery and 2 weeks and 4 weeks after the surgery.Results(1)Four weeks after the surgery,the overall effective rate of the observation group was 92.00%(23/25)and that of the control group was 80.00%(20/25).The intergroup comparison(tested by rank sum test)showed that the efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)The shoulder pain VAS scores of patients in both groups were significantly decreased 2 and 4 weeks after the surgery in comparison with those before surgery(P<0.05),and the reduction in VAS scores in the observation group was significantly superior to that in the control group 2 and 4 weeks after the surgery(P<0.05).(3)The should function UCLA scores of both groups were significantly increased 2 and 4 weeks after the surgery compared with the preoperative scores(P<0.05),and the effect of the observation group on the increase of the UCLA scores 2 and 4 weeks after surgery was significantly superior to that of the control group(P<0.05).Conclusion Wet compress with compound Huanglian Lotion for patients after arthroscopic acromioplasty can accelerate the elimination of shoulder swelling,effectively relieve post-operative pai
