摘 要:【目的】观察电针联合推拿对卒中大鼠认知功能障碍的治疗作用及机制。【方法】将18只SD大鼠随机分为假手术组、模型组和治疗组,每组6只。模型组和治疗组大鼠建立大脑中动脉栓塞(MCAO)模型。治疗组大鼠给予电针百会、足三里穴联合点按百会、神庭、足三里穴治疗。治疗结束后,通过滚轴试验与网格试验对各组大鼠进行行为学评估,莫里斯水迷宫试验对各组大鼠认知功能进行评价,采用2,3,5-氯化三苯基四氮唑(TTC)染色法观察各组大鼠脑梗死情况,采用实时定量聚合酶链反应(qRT-PCR)法检测各组大鼠小胶质细胞M1/M2的极化情况。【结果】与假手术组比较,模型组大鼠神经学评分显著下降,逃避潜伏期显著延长,穿越平台次数降低,第一次达到平台的时间增加,停留在目标象限的时间缩短,脑梗死面积增加,小胶质细胞M1极化标志物增多,M2极化标志物减少(均P<0.01);与模型组比较,治疗组上述结果均得到不同程度逆转(均P<0.01)。【结论】电针联合推拿可改善卒中大鼠认知功能障碍,其机制与促进小胶质细胞M2极化有关。Objective To observe the therapeutic effects and mechanisms of electro-acupuncture combined with tuina in treating stroke rats with cognitive dysfunction.Methods Eighteen SD rats were randomly divided into sham-operation group,model group and treatment group,with 6 rats in each group.A middle cerebral artery occlusion(MCAO)model was established in the model group and treatment group.The rats in the treatment group were given electro-acupuncture to Baihui(GV20)and Zusanli(ST36)combined with point pressure on Baihui(GV20),Shenting(GV24)and Zusanli(ST36).After treatment,the rats were evaluated behaviorally by rotarod test and grid test,cognitive function was evaluated by Morris water maze test,cerebral infarction was observed by 2,3,5-triphenyltetrazolium chloride(TTC)staining,and M1/M2 polarization of microglia in each group was detected by quantitative real-time polymerase chain reaction(qRT-PCR).Results Compared with the shamoperation group,the neurological scores were significantly decreased,escape latency was prolonged,platform crossing times was significantly decreased,time to reach the platform for the first time was significantly increased,time to stay in the target quadrant was significantly shortened,cerebral infarction area was significantly increased,M1 polarization markers of microglia were increased,M2 polarization markers were decreased in the model group(all P<0.01).Compared with the model group,the above results were reversed in the treatment group to varying degrees(all P<0.01).Conclusion Electro-acupuncture combined with tuina improves cognitive dysfunction by a mechanism related to promotion of microglia M2 polarization in stroke rats.
