摘　　要：正骨手法复位结合小夹板固定治疗桡骨远端骨折具有鲜明的中医特色。该文对林定坤教授治疗桡骨远端骨折的正骨手法和小夹板固定方法进行了介绍。林定坤教授治疗桡骨远端骨折的正骨手法在传承岭南何氏骨伤及上海石氏伤科正骨手法的基础上,有一定的改良与创新,具体体现在:(1)手法复位时以尺骨茎突为轴,向尺侧旋转拔伸腕关节,恢复桡骨高度和尺偏角。(2)用腕关节的远侧关节面,回旋挤压近端关节面以达到复位关节面的效果。(3)针对骨折时腕部肌腱卡压在骨折端或抵在尖锐的骨折端周围的患者,提出实施理筋手法。(4)针对患者的不同骨折类型、皮肤情况,分别采用护肤垫、维稳垫、填充垫等压垫,提高固定的稳定性、舒适性。The treatment of distal radius fractures by orthopaedic manipulations combined with small splint fixation has distinctive Chinese medical features.This article introduces the methods of orthopaedic manipulations and small splint fixation established by Professor LIN Ding-Kun for the treatment of distal radius fractures.LIN’s orthopedic manipulations inherited the methods of Lingnan HE’s Traumatology and the Shanghai SHI’s Traumatology with certain improvements and innovations,which manifest as the following:(1)the ulnar styloid process is used as the axis during repositioning,and the wrist joint is rotated and stretched toward the ulnar side to restore the radial height and ulnar inclination angle.(2)By making the distal articular surface of the wrist gyrate and squeeze the proximal articular surface,the reposition of the articular surface is achieved.(3)The application of the manipulations for soothing tendons is proposed for patients with fractures where the wrist tendon is trapped around the fracture end or against a sharp fracture end.(4)For patients with different fracture types and skin conditions,pressure pads such as skin-care pads,stabilizing pads and padding pads are recommended to improve the stability and comfort of the fixation.
