张仲景“治未病”思想在甲状腺结节防治中的应用探讨

摘　　要：甲状腺结节属于中医“瘿病”范畴,其发病受情志、饮食、体质等因素的影响,与肝、脾密切相关。木郁、土湿则肝郁气滞,脾失健运,最终导致痰、气、瘀三者相合壅结颈前成患。张仲景的《伤寒论》继承并发展了《黄帝内经》“治未病”思想。结合张仲景治未病思想,甲状腺结节的防治当从以下三方面进行:一是未病先防,通过节饮食、调情志、顺天时以阻止疾病发生;二是既病防变,从肝调治、从脾调治以截断病势,防止其传变与恶化;三是瘥后防复,做到静息调养、合理饮食等以阻断疾病复发。张仲景“治未病”思想是中医防治疾病思想中的精华,土木同调可为甲状腺结节的防治提供新思路,对运用中医药防治甲状腺结节具有指导意义。Thyroid nodules can be classified into the category of“goiter disease”in the field of traditional Chinese medicine(TCM).The onset of thyroid nodules is influenced by emotions,diet and body constitution,and is closely related to the liver and spleen.The depression of the liver and the dampness due to spleen deficiency will result into liver constraint and qi stagnation as well as failure of spleen in the transportation,which eventually leads to the accumulation of combined phlegm,qi and blood stasis in the front part of the neck.ZHANG Zhong-Jing’s Shang Han Lun(Treatise on Exogenous Febrile Diseases)inherited and developed the thoughts of“preventive treatment of disease”from Huang Di Nei Jing(Huangdi’s Cannon of Medicine).By following ZHANG Zhong-Jing’s philosophy of preventive treatment of disease,the prevention and treatment of thyroid nodules should start from the following 3 aspects:firstly,prevention before disease onset can be achieved through regular diet,regulation of emotions,and adaption to the climate to prevent the occurrence of disease;the second is to prevent the progress of diseases by regulating the liver and the spleen to counteract the disease and to prevent disease from exacerbating;the third is to prevent the disease from recurring after the cure of diseases by taking good rest and preserving health and having proper diet to prevent the disease from recurring.ZHANG Zhong-Jing’s thoughts of“preventive treatment of disease”is the essence of TCM theory for preventing and treating diseases,and the method of harmonization of earth(spleen)and wood(liver)can provide new ideas for the prevention and treatment of thyroid nodules,which is of guiding significance for the prevention and treatment of thyroid nodules with Chinese medicine.
