国医大师王庆国教授以水心理论治疗扩张型心肌病心力衰竭经验总结

摘　　要：扩张型心肌病心力衰竭属于临床疑难疾病,目前病因尚不明确,西医对其无特异性治疗方法,多以控制心力衰竭和心律失常,预防猝死和栓塞为主,病死率较高。而中医对于扩张型心肌病心力衰竭独特的认识和辨治方法,为临床治疗提供了新的方向。王庆国教授认为其核心病机为阳虚水泛,心阳不足而为先,脾肾软弱不制寒,痰饮、瘀血、水湿之邪上犯而成扩张型心肌病心力衰竭。对此,王教授临床注重中西汇参,辨病与辨证结合,以水心理论指导治疗思路,补气通阳为先,温补脾肾制寒,调和气血阴阳。此外,王教授临床常常于方中佐入连翘,可纠偏防热,实有画龙点睛之妙。从临床效果来看,中医药对扩张型心肌病心力衰竭不仅可以从功能上改善,更可以从结构上改善,疗效显著。Heart failure due to dilated heart disease is a clinical difficult disease,and its etiology remains unclear.Western medicine has no specific treatment method for it.Western medicine mainly focuses on controlling the heart failure and arrhythmia,to avoid sudden death and embolism,but there is a high mortality rate.Traditional Chinese medicine’s unique understanding and treatment of heart failure caused by dilated heart disease provides a new direction for clinical treatment.Professor WANG Qingguo believes that its core pathogenesis is edema due to yang deficiency.The yang deficiency of heart comes first,the weakness of the spleen and kidney does not control the coldness,and the abnormal of phlegm,blood stasis,and water dampness is committed,forming heart disease and heart failure.As for this,Professor WANG Qingguo pays attention to the combination of traditional Chinese medicine and western medicine,the disease and syndrome differentiation,and adopts the water-core theory to guide the treatment method.Replenish qi and clear yang first,and warm the spleen and kidney to control coldness,and harmonize qi and blood yin and yang.In addition,Professor WANG Qingguo prefers to add Forsythia suspense to the prescription in clinical practice,which can correct the deviation and prevent the fever,which is regarded as the highlight in the prescription.From the perspective of clinical effect,traditional Chinese medicine can not only improve the function of preventing the heart disease and heart failure,but also ameliorate the structure of heart.
