摘  要:目的采用非靶向代谢组学技术研究血热妄行证过敏性紫癜(HSP)患儿经加味水牛角地黄汤治疗后内源性物质的变化。方法按纳入和排除标准,收集血热妄行证HSP患儿63例为试验组,健康体检儿童50例为健康组。收集试验组患儿治疗前后及健康组儿童早晨尿液标本,应用超高效液相色谱-四级杆飞行时间质谱(UPLC-Q-TOF/MS)技术结合Progenesis QI软件分析HSP患儿经加味水牛角地黄汤治疗后尿液中差异性生物标志物。结果经试验组治疗前与健康组对比分析,鉴定得到98个差异生物标志物,其中硫酸盐、磷脂酰乙醇胺显著上调(P<0.05),经加味水牛角地黄汤治疗后有显著下降(P<0.05);L-组氨酸、L-色氨酸、6-羟基褪黑激素、1,1-二氯乙烯显著下调(P<0.05),经加味水牛角地黄汤治疗后有显著升高(P<0.05)。主要涉及5条代谢通路,分别为组氨酸代谢通路、硫代谢通路、戊糖和葡萄糖醛酸的相互转化代谢通路、色氨酸代谢通路、甘油磷脂代谢通路。结论初步鉴定了加味水牛角地黄汤治疗血热妄行证HSP前后的差异性代谢标记物,在代谢组学层面阐明了加味水牛角地黄汤发挥疗效的可能作用机制。Objective Nontargeted metabonomics was used to study the changes of endogenous substances in children with complication of Henoch-Schönlein purpura(HSP)with blood-heat inducing extravasation of blood syndrome before and after treatment with modified Shuiniujiao Dihuang Decoction.Methods According to the inclusion and exclusion criteria,63 cases of HSP with blood heat inducing extravasation of blood syndrome were collected as the trial group and 50 cases of healthy children were set as the health group.Morning urine samples were collected from children of all groups before and after treatment.Ultra-high performance liquid chromatographyquadrupole time-of-flight mass spectrometry(UPLC-Q-TOF/MS)combined with Progenesis QI software was used to analyze the differential biomarkers in urine samples of HSP children with blood-heat inducing extravasation of blood syndrome before and after treatment with modified Shuiniujiao Dihuang Decoction.Results Compared with the health group,98 differential biomarkers were identified in the trial group before treatment.Sulfate,phosphatidylethanolamine were significantly up-regulated(P<0.05),but significantly decreased after treatment with modified Shuiniujiao Dihuang Decoction(P<0.05).L-Histidine,L-tryptophan,6-hydroxy melatonin and 1,1-dichloroethylene were significantly down-regulated(P<0.05),but significantly increased after treatment with modified Shuiniujiao Dihuang Decoction(P<0.05).It has been found that 5 major metabolic pathways involved in HSP with blood heat inducing extravasation of blood syndrome,including histidine metabolic pathway,thiometabolic pathway,pentose and glucuronic acid conversion metabolic pathway,tryptophan metabolic pathway,and glycerol phospholipid metabolic pathway.Conclusion The differential metabolic markers before and after the treatment of complication of HSP and blood heat inducing extravasation of blood syndrome with modified Shuiniujiao Dihuang Decoction were preliminarily identified.At the level of metabolomics,the possible mechanism of modifie
