摘  要:目的:探讨256排能谱CT冠状动脉成像对不同心率患者单个心动周期中应用的可行性,并对图像质量及辐射剂量进行评价。方法:前瞻性连续收集2021年4月至2021年12月在医院行256层能谱CT冠状动脉成像的127例患者,根据扫描的实时心率将其分为<65次/min组(56例)、65~100次/min组(41例)和>100次/min组(30例)。比较3组之间冠状动脉CT血管造影(CCTA)的图像质量及辐射剂量;比较冠状动脉追踪冻结(SSF)技术和标准(STD)算法两种不同重建方法的图像质量;采用Kappa检验分析两名放射科医师对图像质量评价的一致性。结果:两名放射科医师对图像质量进行的分析结果一致性较好(Kappa=0.895,P<0.001)。<65次/min组图像的质量评分(4.78±0.34)分,高于65~100次/min组和>100次/min组,差异具有统计学意义(t=8.756,t=12.365;P<0.05)。<65次/min组、65~100次/min组和>100次/min组应用SSF算法重建图像质量明显优于STD重建图像质量,且差异具有统计学意义(Z=3.606,Z=3.162,Z=2.828;P<0.05)。<65次/min组、65~100次/min组和>100次/min组有效辐射剂量中位数分别为1.83 mSv、2.06 mSv和2.46 mSv,且3组有效辐射剂量差异无统计学意义(F=2.357,F=2.875,F=2.218;P>0.05)。结论:256层能谱CT在不同心率患者采用单个心动周期冠状动脉成像具有可行性,且可降低心率>100次/min患者的辐射剂量,在不同心率患者中采用SSF技术均可以显著提高图像质量。Objective:To explore the feasibility of 256-slice spectral coronary computed tomography angiography(CCTA)in patients with different heart rates in a single cardiac cycle,and to evaluate the image quality and radiation dose of that.Methods:From April 2021 to December 2021,a total of 127 patients who underwent 256-slice spectral CCTA in the hospital were prospectively and continuously collected,and they were divided into“<65 times/min”group(56 cases)and“65-100 times/min”group(41 cases)and“>100 times/min”group(30 cases)according to the realtime heart rate of scan.The image quality and radiation dose of CCTA were compared among the three groups,and the image qualities of two different reconstruction methods that respectively were snap-shot freeze(SSF)technology and standard(STD)algorithm were compared.The Kappa test was adopted to analyze the consistency of two radiologists in evaluating image quality.Results:The consistency of the analytic results of two radiologists for image quality were better(Kappa=0.895,P<0.001).The quality score(4.78±0.34)of the image of the“<65 times/min”group was significantly higher than that of the“65-100 times/min”group and the“>100 times/min”group(t=8.756,t=12.365,P<0.05),respectively.The reconstructed image qualities of“<65 times/min”group,“65-100 times/min”group and the“>100 times/min”group as SSF algorithm were significantly better than reconstructed those as STD(Z=3.606,Z=3.162,Z=2.828,P<0.05),respectively.The median of effective radiation doses of“<65 times/min”group,“65-100 times/min”group and“>100 times/min”group respectively were 1.83 mSv,2.06 mSv and 2.46 mSv,and there were no significant differences in effective radiation dose among the three groups(F=2.357,F=2.875,F=2.218,P>0.05).Conclusion:It is feasibility that 256-slice spectral CT adopts coronary artery imaging in single cardiac cycle for patients with different heart rates,which can reduce the radiation dose of patients whose heart rates are larger than 100 times/min.Using
