彩色多普勒定位辅助阔筋膜张肌皮瓣在股前外侧皮瓣穿支变异时足踝部皮肤软组织缺损修复的研究

摘　　要：目的:探讨彩色多普勒定位辅助阔筋膜张肌皮瓣和股前外侧穿支皮瓣修复足踝部皮肤软组织缺损的修复效果和预后情况。方法:选取医院收治的36例足踝部创伤患者,根据皮瓣修复方式不同,将其分为观察组(14例)和对照组(22例),观察组应用彩色多普勒定位辅助阔筋膜张肌皮瓣修复,对照组应用彩色多普勒定位辅助股前外侧穿支皮瓣,分析患者彩色多普勒下蒂部血管直径和血流参数,手术时间,皮瓣修复效果和足踝部功能恢复情况。结果:观察组血管直径、最大流速(V_(max))及平均流速(V_(mean))明显高于对照组(t=3.882,t=3.016,t=3.152;P<0.05),手术时长明显低于对照组,差异有统计学意义(t=-3.016,P<0.05)。两组血管危象及总成活率及术后3个月两组踝关节功能各项比较差异均无统计学意义。结论:彩色多普勒定位辅助阔筋膜张肌皮瓣可作为股前外侧皮瓣穿支变异时的替代皮瓣对足踝部皮肤软组织缺损进行修复,具有手术时间短、并发症发生率低及患者足踝部功能恢复良好的优点。Objective:To analyze the repaired effect and prognosis of the assisted tensor fascia latae muscle flap and the perforating branch of anterolateral thigh flap with color Doppler ultrasound localization in repairing the defects of skin and soft tissue of foot and ankle.Methods:The patients with foot and ankle trauma who admitted to hospital were selected,and 14 patients with the assisted tensor fascia latae muscle flap by color Doppler ultrasound localization were selected as the observation group,and 22 patients with the assisted perforating branch of anterolateral thigh flap by color Doppler ultrasound localization were selected as the control group at the same time.The vascular diameter and the parameters of blood flow in the inferior peduncle,as well as the surgical time,flap repair effect and the recovery of the function of foot and ankle of Doppler ultrasound of patients were retrospectively analyzed.Results:The vascular diameter,V_(max) and V_(mean) of the observation group were significantly higher than those in the control group(t=3.882,t=3.016,t=3.152,P<0.05),and the surgical duration of the observation group was significantly lower than that of the control group and the difference was statistically significant(t=-3.016,P<0.05),respectively.There were no statistical significance in the vascular crisis and total survival rate,as well as the function of ankle joint at the 3rd month after surgery between two groups(P>0.05).Conclusion:The assisted tensor fascia latae muscle flap with localization of color Doppler ultrasound can be used as the replacement flap to repair the defect of the soft tissue of skin of foot and ankle when the perforating branch of anterolateral thigh flap occurs variation,which advantages include shortly surgical time,low incidence of complication and favorable recovery of the function of foot and ankle of patients.
