子痫前期孕妇血清circRNA VRK1表达水平与临床指标的相关性及对妊娠结局的预测价值

摘　　要：目的探讨子痫前期(preeclampsia,PE)孕妇血清环状RNA(circRNA)牛痘相关激酶1(VRK1)表达水平与临床指标的相关性及其对妊娠结局的预测价值。方法选取2019年1月至2021年12月邯郸市妇幼保健院就诊的95例PE孕妇为PE组,纳入同期来院检查的100例健康孕妇为对照组。记录两组孕妇临床指标及妊娠结局,并检测血清circRNA VRK1表达水平。采用Pearson相关性分析探讨circRNA VRK1与舒张压(DBP)、尿酸(UA)等临床指标的相关性。ROC曲线分析血清circRNA VRK1表达水平对PE孕妇不良妊娠结局的预测价值。结果对照组血清circRNA VRK1表达水平(0.97±0.18)低于PE组(1.83±0.50)(P<0.05)。PE组收缩压(SBP)、DBP、24 h尿蛋白、尿酸氮(BUN)、肌酐(Scr)、超敏-C反应蛋白(Hs-CRP)、UA、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)显著高于对照组(P<0.05)。circRNA VRK1与SBP、DBP、24 h尿蛋白、BUN、Hs-CRP、ALT、AST存在正相关(r=0.591、0.546、0.617、0.482、0.450、0.594、0.536,P<0.05)。总不良妊娠结局发生率PE组(50.53%)高于对照组(13.00%)(P<0.05)。PE组不良妊娠结局孕妇血清circRNA VRK1表达水平(2.15±0.52)显著高于正常妊娠结局者(1.50±0.48)(P<0.05)。circRNA VRK1预测不良妊娠结局的曲线下面积为0.857(95%CI:0.779-0.934)。结论PE孕妇血清circRNA VRK1呈上调表达,且与临床特征变化相关,对不良妊娠结局有较高预测效能。Objective To investigate the correlation between the expression level of serum circular RNA(circRNA)vacciniarelated kinase 1(VRK1)and clinical indicators in pregnant women with preeclampsia(PE)and its predictive value for pregnancy outcome.Methods 95 Cases of PE pregnant women who visited Handan Maternal and Child Health Hospital from January 2019 to December 2021 were selected as PE group,and 100 healthy pregnant women who came to the hospital for examination during the same period were included as control group.The clinical indicators and pregnancy outcomes of the two groups were recorded,and the serum circRNA VRKI expression level was detected.Pearson correlation analysis was used to investigate the correlation between circRNA VRKI and diastolic blood pressure(DBP),uric acid(UA),and other clinical indicators.The value of serum circRNA VRKI expression level in predicting adverse pregnancy outcomes in PE pregnant women was analyzed by ROC curve.Results The expression level of serum circRNA VRK1 in the control group(0.97±0.18)was lower than that in the PE group(1.83±0.50)(P<0.05).The systolic blood pressure(SBP),DBP,24 h urine protein,uric acid nitrogen(BUN),creatinine(Scr),high-sensitivity-C-reactive protein(Hs-CRP),UA,alanine aminotransferase(ALT),and aspartate aminotransferase(AST)in the PE group were significantly higher than those in the control group(P<0.05).There was a positive correlation between circRNA VRK1 and SBP,DBP,24 h urine protein,BUN,Hs-CRP,ALT,AST(r=0.591,0.546,0.617,0.482,0.450,0.594,0.536,P<0.05).The incidence of total adverse pregnancy outcome in PE group(50.53%)was higher than that in control group(13.00%)(P<0.05).The expression level of circRNA VRK1 in serum of pregnant women with adverse pregnancy outcomes in PE group(2.15+0.52)was significantly higher than that in women with normal pregnancy outcomes(1.50±0.48)(P<0.05).The area under the curve of circRNA VRK1 for predicting adverse pregnancy outcomes was 0.857(95%CI:0.779-0.934).Conclusion The expression of serum circRNA VRK1 in PE pr
