精子DNA碎片指数与精液常规参数和精子运动参数的相关性分析

摘　　要：目的探讨精子DNA碎片指数(DFI)与精液常规参数以及精子运动参数之间的相关性。方法回顾性分析2021年3月至2022年2月在福建省人类精子库男科实验室进行精液检查的288例男性患者资料,根据DFI值分为DFI≤15%(46.53%)、15%<DFI<30%(36.46%)、DFI≥30%(17.01%)3组,分别比较3组的年龄、精液常规参数和精子运动学参数之间是否存在差异,并分析精子DFI与各精液参数的相关性。结果3组组间比较结果显示,年龄、禁欲时间、精液量、精子水平、PR级精子百分比、精子总活力、精子运动参数、形态正常精子百分比之间的差异均有统计学意义(P<0.01),相关性分析表明,精子DFI与年龄、禁欲时间、精液量之间存在显著正相关(r=0.269、0.157、0.163,P<0.01),精子DFI与PR级精子百分比、精子总活力、精子运动参数(VCL、VSL、VAP、ALH、BCF、MAD、LIN、WOB、STR)、形态正常精子百分比之间存在显著负相关(r=-0.438、-0.443、-0.427、-0.416、-0.428、-0.306、-0.428、-0.442、-0.307、-0.307、-0.318、-0.255,P<0.01)。多元线性回归分析结果表明精子总活力、年龄、精液量、形态正常精子百分比和禁欲时间是精子DFI相关的5个变量,标准化回归系数分别为-0.370、0.195、0.151、-0.116、0.111。结论精子DFI与患者年龄、精液常规参数包括精子运动参数之间存在相关性,可综合多参数进行男性精子质量评估。Objective To explore the relationship of sperm DNA fragmenation index(DFI)with semen parameters and sperm motion parameters.Methods The data of 288 male patients who underwent semen examination in the andrology laboratory of Fujian Human Sperm Bank from March 2021 to February 2022 were retrospectively analyzed.According to the DFI value,they were divided into three groups:DFI≤15%(46.53%),15%<DFI<30%(36.46%),and DFI≥30%(17.01%).The differences in age,semen parameters and sperm motion parameters of the three groups were compared,and the correlation between sperm DFI and semen parameters was analyzed.Results The results of comparison among the three groups showed that there were significant differences among age,abstinencetime,semenvolume,sperm concentration,percentage of progressive motile sperm,total sperm motility,sperm motion parameters and percentage of morphologically normal sperm(P<0.01).Correlation analysis showed that there was a significant positive correlation between sperm DFI and age,abstinence time and semen volume(r=0.269,0.157,0.163,P<0.01).There was a significant negative correlation between DFI and percentage of progressive motile sperm,total sperm motility,sperm motion parameters(VCL,VSL,VAP,ALH,BCF,MAD,LIN,WOB,STR),percentage of morphologically normal sperm(r=-0.438,-0.443,-0.427,-0.416,-0.428,-0.306,-0.428,-0.442,-0.307,-0.307,-0.318,-0.255,P<0.01).The results of multiple linear regression analysis showed that total sperm motility,age,semen volume,percentage of morphologically normal sperm and abstinence time were five variables related to sperm DFI.The standardized regression coefficients were-0.370,0.195,0.151,-0.116 and 0.111.Conclusion There is a correlation between sperm DFI and age,semen parameters including sperm motionparameters,which can be used to evaluate male sperm quality.
