补肾调冲汤治疗围绝经期排卵障碍性异常子宫出血35例

摘　　要：目的:观察补肾调冲汤治疗围绝经期排卵障碍性异常子宫出血(ovulatory dysfunction abnormal uterine bleeding,AUB-O)的临床疗效。方法:将70例围绝经期AUB-O患者按照随机数字表法分为对照组和观察组,每组各35例。对照组给予地屈孕酮片口服,观察组在对照组治疗的基础上给予补肾调冲汤治疗。比较两组患者治疗前后中医证候积分、月经失血图评分(pictorial blood loss assessment chart,PBAC)、子宫内膜厚度、激素水平[促卵泡生成素(follicle stimulating hormone,FSH)、黄体生成素(luteinizing hormone,LH)、雌二醇(estradiol,E_(2))]变化情况。结果:两组患者治疗后阴道出血、神疲懒言、面色胱白及舌淡苔薄等中医证候评分低于本组治疗前,且治疗后观察组低于对照组(P<0.05)。两组患者治疗后PBAC评分、子宫内膜厚度低于本组治疗前,且治疗后观察组低于对照组(P<0.05)。两组患者治疗后E_(2)水平高于本组治疗前,LH和FSH低于本组治疗前,且治疗后组间比较,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:补肾调冲汤治疗围绝经期AUB-O,可改善患者的临床症状,减少出血量,调节激素水平。Objective:To observe the clinical efficacy of Kidney-Tonifying and TV-Regulating Decoction on perimenopausal-period ovulatory dysfunction abnormal uterine bleeding(AUB-O).Methods:A total of 70 perimenopausal-period AUB-O patients were randomly divided into the control group and the observation group according to the random number table method,with 35 cases in each group.The control group was given oral administration of Dydrogesterone Tablets,while the observation group was treated with Kidney-Tonifying and TV-Regulating Decoction on the basis of the control group.The changes in TCM syndrome scores,menstrual blood loss assessment chart(PBAC),endometrial thickness,hormone levels [including follicle stimulating hormone(FSH),luteinizing hormone(LH),and estradiol(E_(2))] between two groups before and after treatment were compared.Results:After treatment,the TCM syndrome scores,such as vaginal bleeding,mental fatigue,lazy speech,pale complexion,and thin tongue coating in both groups were lower than those before treatment,and the observation group's scores were lower than those of the control group(P<0.05);The PBAC score and endometrial thickness of the two groups were lower than those before treatment,and the observation group's above indexes were lower than those of the control group(P<0.05);The E_(2) level of the two groups was higher than that before treatment,while LH and FSH were lower than those before treatment.All the differences between the two groups were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Kidney-Tonifying and TV-Regulating Decoction can improve the clinical symptoms of patients with perimenopausal-period AUB-O,reduce bleeding,and regulate hormone levels.
