益肾化痰方联合来曲唑片治疗肾虚痰湿型多囊卵巢综合征所致不孕症56例

摘　　要：目的:观察益肾化痰方联合来曲唑治疗肾虚痰湿型多囊卵巢综合征(polycystic ovary syndrome,PCOS)所致不孕症患的临床疗效。方法:将112例肾虚痰湿型PCOS所致不孕症患者按照随机数字表法分为联合组与来曲唑组,每组各56例。来曲唑组给予来曲唑片治疗,联合组在来曲唑组治疗的基础上联合益肾化痰方治疗。比较两组患者的临床疗效、临床妊娠率、自然流产率及治疗前后中医证候积分、雌二醇(estradiol,E_(2))、促卵泡生成素(follicle-stimulating hormone,FSH)、睾酮(testosterone,T)、促黄体生成素(luteinizing hormone,LH)、血清单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)、瘦素(leptin,LP)、游离脂肪酸(free fatty acid,FFA)、类胰岛素样生长因子1(insulin-like growth factor-1,IGF-1)水平变化情况。结果:两组患者治疗后主症、次症积分及总分低于本组治疗前,且治疗后联合组低于来曲唑组(P<0.05)。联合组有效率为94.64%,来曲唑组有效率为82.14%,两组患者有效率比较,差异有统计学意义(P<0.05)。两组患者治疗后E_(2)、T、LH、LH/FSH、MCP-1、LP、FFA、IGF-1、FGF-21水平低于本组治疗前,且联合组较来曲唑组降低(P<0.05)。联合组临床妊娠率高于来曲唑组(P<0.05),两组自然流产率比较,差异无统计学意义(P>0.05)。结论:益肾化痰方联合来曲唑片治疗肾虚痰湿型PCOS所致不孕症,可提高临床妊娠率,改善临床症状,调节激素水平。Objective:To observe the clinical eficacy of Kidney-Replenishing and Phlegm-Resolving Formula combined with Letrozole Tablets on infertility caused by polycystic ovarian syndrome(PCOS)of kidney deficiency and phlegm dampness type.Methods:A total of 112 cases of infertility caused by PCOS with kidney deficiency and phlegm dampness syndrome were randomly divided into the combined group and the letrozole group,with 56 cases in each group.The letrozole group was treated with Letrozole Tablets,while the combined group was treated with Kidney-Replenishing and Phlegm-Resolving Formula on the basis of the letrozole group.The clinical efficacy,clinical pregnancy rate,natural abortion rate,and changes in TCM syndrome scores,levels of estradiol(E_(2)),follicle stimulating hormone(FSH),testosterone(T),luteinizing hormone(LH),serum monocyte chemoattractant protein-1(MCP-1),leptin(LP)free fatty acid(FFA)and insulin-like growth factor-1(IGF-1)between two groups before and after treatment were compared.Results:After treatment,the main and secondary symptom scores and total scores of the two groups were lower than those before treatment,and the combined group's above scores were lower than those of the letrozole group(P<0.05).The effective rate of the combined group was 94.64%,while that of the letrozole group was 82.14%.The difference was statistically significant(P<0.05).After treatment,the levels of E_(2),T,LH,LH/FSH,MCP-1,LP,FFA,IGF-1,and FGF-21 in both groupswere lower than those before treatment,and the combined group's above levels were lower than those of the letrozole group(P<0.05).The clinical pregnancy rate of the combined group was higher than that of the letrozole group(P<0.05),but there was no statistically significant difference in the spontaneous abortion rate between the two groups(P>0.05).Conclusion:The combination of Kidney-Replenishing and Phlegm-Resolving Formula and Letrozole can improve the clinical pregnancy rate,enhance clinical symptoms,and regulate hormone levels in the treatment of infertility caused b
