闭合复位克氏针内固定治疗儿童肱骨髁上骨折的效果  

Effect of Closed Reduction and Kirschner Wire Internal Fixation in the Treatment of Supracondylar Fracture of Humerus in Children

摘　　要：目的:探讨对儿童肱骨髁上骨折患儿采用闭合复位克氏针内固定治疗的效果。方法:选取2020年1月—2022年3月毕节市中医医院收治的93例儿童肱骨髁上骨折患儿作为研究对象进行研究,骨折类型为GartlandⅡ型、Ⅲ型,根据复位方式的不同将其进行分组,其中切开复位组42例,手术方式为切开复位+克氏针内固定;闭合复位组51例,手术方式为闭合复位+克氏针内固定。比较两组手术时间、术中出血量、住院时间、骨折愈合时间、术后6个月肘关节功能恢复优良率、关节功能指标和术后并发症情况。结果:闭合复位组手术时间和住院时间较切开复位组缩短,术中出血量较切开复位组减少,差异有统计学意义(P<0.05)。两组术后6个月肘关节功能恢复优良率、骨折愈合时间及术后6个月肘关节活动度、提携角、Baumann角、提携角丢失比较,差异无统计学意义(P>0.05);两组内翻畸形、钉道感染、尺神经损伤发生率及并发症总发生率比较,差异无统计学意义(P>0.05)。结论:闭合复位克氏针内固定治疗儿童肱骨髁上骨折在促进骨折愈合和关节功能恢复方面能达到与切开复位内固定相当的效果,但前者在降低手术创伤和缩短住院时间上更有优势。Objective:To investigate the effect of closed reduction and Kirschner wire internal fixation in the treatment of supracondylar fracture of humerus in children.Method:A total of 93 children with supracondylar fracture of humerus who were treated Bijie Traditional Chinese Medicine Hospital from January 2020 to March 2022 were selected as the study subjects.The fracture type was GartlandⅡ,GartlandⅢ.According to the different reduction methods,they were divided into two groups:the open reduction group(42 cases),and the operation method was open reduction+Kirschner wire internal fixation,the closed reduction group(51 cases),and the surgical method was closed reduction+Kirschner wire internal fixation.Operation time,intraoperative blood loss,length of hospital stay,fracture healing time,excellent and good rate of elbow joint functional recovery at 6 months after surgery,joint function index and postoperative complications were compared between the two groups.Result:The surgical time and hospitalization time in the closed reduction group were shorter than those in the open reduction group,and the intraoperative bleeding was reduced compared with that in the open reduction group,the differences were statistically significant(P<0.05).There were no statistically significant differences in the excellent and good rates of elbow joint function recovery at 6 months after surgery,fracture healing time,and elbow joint mobility,lifting angle,Baumann angle,and loss of lifting angle at 6 months after surgery between two groups(P>0.05);there were no significant differences in the incidence of varus deformity,nail infection,ulnar nerve injury and the total incidence of complications between two groups(P>0.05).Conclusion:Closed reduction and Kirschner wire internal fixation for children's supracondylar of humerus fracture can achieve the same effect as open reduction and internal fixation in promoting fracture healing and joint function recovery,but the former has more advantages in reducing surgical trauma and shortening hospital
