阴道试产与选择性剖宫产治疗边缘性前置胎盘的临床效果及安全性分析

摘　　要：目的研究边缘性前置胎盘治疗过程中应用阴道试产与选择性剖宫产的临床效果及安全性。方法选取78例边缘性前置胎盘患者,将患者按照住院号奇偶数分为研究组与对照组,每组39例。研究组实施阴道试产治疗,对照组实施选择性剖宫产治疗。对比两组治疗后新生儿Apgar评分、产后出血量及不良结局发生率情况。结果研究组不良结局发生率5.13%显著低于对照组的28.21%,差异有统计学意义(P<0.05)。研究组产后2 h、24 h出血量少于对照组,差异有统计学意义(P<0.05);两组新生儿1 min的Apgar评分6分、9分占比对比,差异无统计学意义(P>0.05)。结论阴道试产与选择性剖宫产治疗效果显著,与后者相比,前者止血效果显著,可以在一定程度上抑制相关不良结局,具有较高的治疗及预后价值。Objective To study the clinical effect and safety of vaginal trial delivery and elective cesarean section in the treatment of marginal placenta previa.Methods 78 patients with marginal placenta previa were selected and divided into study group and control group according to the odd and even hospitalization number,with 39 cases in each group.The study group was treated with vaginal trial labor,and the control group was treated with elective cesarean section.Neonatal Apgar score,postpartum blood loss and incidence of adverse outcomes were compared between the two groups.Results The incidence of adverse outcome in the study group(5.13%)was significantly lower than that in the control group(28.21%),and the difference was statistically significant(P<0.05).The postpartum blood loss of 2 h and 24 h in the study group was less than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in Apgar scores of 6 and 9 in 1 min between the two groups(P>0.05).Conclusion Vaginal trial delivery and elective cesarean section have significant therapeutic effects.Compared with the latter,the former has significant hemostatic effect and can inhibit related adverse outcomes to a certain extent,which has higher therapeutic and prognostic value.
