Chiari畸形Ⅰ型合并脊髓空洞症患者并发中枢性睡眠呼吸暂停综合征的护理实践

摘　　要：目的分析Chiari畸形Ⅰ型合并脊髓空洞症患者并发中枢性睡眠呼吸暂停综合征的精细个性化护理效果,并总结护理实践经验。方法回顾性分析天津市环湖医院2020年10月-2022年12月收治的Chiari畸形Ⅰ型合并脊髓空洞症并发中枢性睡眠呼吸暂停综合征患者共计40例为研究对象,所有患者均行全麻下手术治疗,并给予术前及术后精细个性化护理,观察患者术前及术后3个月呼吸暂停频次、累计暂停时间及最低血氧饱和度,并发症发生率。结果患者呼吸暂停频次较术前有明显的减少,累计暂停时间显著下降,最低血氧饱和度较术前有明显上升(P<0.05);并发症总发生率为7.5%(P<0.05)。结论Chiari畸形Ⅰ型合并脊髓空洞症并发中枢性睡眠呼吸暂停综合征患者给予手术联合精细个性化护理治疗效果较好,可改善患者的睡眠呼吸暂停症状,降低并发症发生率,可为治疗及预后提供保障,值得临床应用推广。Objective To analyze the effect of fine individualized nursing care for patients with Chiari malformation type Ⅰ complicated with syringomyelia complicated with central sleep apnea syndrome,and summarize nursing experience.Methods From October 2020 to December 2022,40 cases of Chiari malformation type Ⅰ complicated with syringomyelia complicated with central sleep apnea syndrome were analyzed retrospectively,all patients were operated under general anesthesia and given preoperative and postoperative individualized nursing.The frequency of apnea,the cumulative duration of apnea and the lowest Oxygenation were observed before and 3 months after operation,incidence of complications.Results The frequency of apnea decreased significantly,the cumulative duration of apnea decreased significantly,the lowest Oxygenation increased significantly(P<0.05),and the total incidence of complications was 7.5%(P<0.05).Conclusion The effect of operation combined with fine individualized nursing for the patients with Chiari malformation type I complicated with Syringomyelia and central sleep apnea syndrome is better,and it can improve the sleep apnea symptoms of the patients,reducing the incidence of complications can provide guarantee for treatment and prognosis,and is worthy of clinical application.
