全自动半巢式实时荧光定量PCR诊断肺外结核的临床价值

摘　　要：目的评价研究半巢式实时荧光定量PCR(GeneXpert MTB)技术辅助诊断肺外结核的临床价值。方法收集住院确诊肺外结核患者75例,患者标本同时利用涂片、固体培养和GeneXpert MTB法进行检测,比较其阳性检出率。结果75例肺外结核标本经涂片、固体培养和GeneXpert MTB法检测,其阳性检出率分别为6.67%(5/75)、17.33%(13/75)和38.67%(29/75),三种方法的特异度均为100%。涂片和GeneXpert MTB法结果比较,P=0.000;固体培养和GeneXpert MTB法结果比较,P=0.004。GeneXpert MTB法具有检测含菌量低的标本的优势。GeneXpert MTB法诊断肺外结核具有较高的敏感度(38.67%)、特异度(100%)、灵敏度(66.18%)、阳性预测值(100%)和阴性预测值(57.01%)。结论半巢式实时荧光定量PCR对辅助诊断肺外结核具有较高的临床价值。Objective To evaluate and study the clinical value of semi-nested real-time fluorescent quantitative PCR(GeneXpert MTB)technology in assisting the diagnosis of extrapulmonary tuberculosis.Methods Seventy five patients with confirmed extrapulmonary tuberculosis were collected.The patient specimens were tested by smear,solid culture and GeneXpert MTB method at the same time,and their positive detection rates were compared.Results Seventy five cases of extrapulmonary tuberculosis specimens were detected by smear,solid culture and GeneXpert MTB method,and the positive rates were 6.67%(5/75),17.33%(13/75)and 38.67%(29/75),respectively.The specificity of each method is 100%.The results of smear and GeneXpert MTB were statistically significant,P=0.000,and the results of solid culture and GeneXpert MTB were statistically significant,P=0.004.The GeneXpert MTB method has the advantage of detecting specimens with low bacterial content.GeneXpert MTB method has high sensitivity(38.67%),specificity(100%),sensitivity(66.18%),positive predictive value(100%)and negative predictive value(57.01%)in the diagnosis of extrapulmonary tuberculosis.Conclusion Semi-nested real-time fluorescent quantitative PCR has great clinical value in assisting the diagnosis of extrapulmonary tuberculosis.
