摘　　要：目的分析胰岛素泵持续皮下注射联合饮食指导对妊娠期糖尿病患者糖脂代谢与母婴结局的影响。方法选取2020年1月至2021年10月在我院接受治疗的100例妊娠期糖尿病患者为研究对象,根据治疗方案的不同将其分为对照组和观察组,各50例。两组均给予饮食指导,同时对照组给予常规胰岛素皮下注射治疗,观察组给予胰岛素泵持续皮下注射治疗。比较两组的治疗效果。结果干预后,观察组的空腹血糖(FPG)、餐后2 h血糖(2 h PG)水平及糖化血红蛋白(HbA1c)低于对照组(P<0.05)。干预后,观察组的高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平优于对照组(P<0.05)。干预后,观察组的胱抑素C(Cys-C)、同型半胱氨酸(Hcy)水平低于对照组(P<0.05)。观察组的母婴不良结局总发生率低于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。结论胰岛素泵持续皮下注射联合饮食指导能有效改善妊娠期糖尿病患者的糖脂代谢水平,改善母婴结局。Objective To analyze the effects of continuous subcutaneous injection of insulin pump combined with dietary guidance on glucose and lipid metabolism and maternal-infant outcomes in patients with gestational diabetes mellitus.Methods A total of 100 patients with gestational diabetes mellitus who were treated in our hospital from January 2020 to October 2021 were selected as the research objects.According to the different treatment plans,the patients were divided into control group and observation group,with 50 cases in each group.Both groups were given dietary guidance,at the same time,the control group was given routine subcutaneous injection of insulin,and the observation group was given continuous subcutaneous injection of insulin pump.The therapeutic effects of the two groups were compared.Results After intervention,the levels of fasting plasma glucose(FPG),2 hour postprandial blood glucose(2 h PG)and glycosylated hemoglobin(HbA1c)in the observation group were lower than those in the control group(P<0.05).After intervention,the levels of high density lipoprotein cholesterol(HDL-C),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)in the observation group were better than those in the control group(P<0.05).After intervention,the levels of cystatin C(Cys-C)and homocysteine(Hcy)in the observation group were lower than those in the control group(P<0.05).The total incidence of maternal-infant adverse outcomes in the observation group was lower than that in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Conclusion Continuous subcutaneous injection of insulin pump combined with dietary guidance can effectively improve the levels of glucose and lipid metabolism in patients with gestational diabetes mellitus,and improve maternal-infant outcomes.
