摘 要:目的 探讨原发性骨质疏松患者血清小而密低密度脂蛋白与骨密度之间的相关性。方法 采用回顾性研究,纳入121例原发性骨质疏松患者,收集其血清小而密低密度脂蛋白浓度、骨密度值、骨代谢指标,三者之间进行简单线性回归分析,并将年龄、体重作为控制变量再次进行偏相关性分析。结果 总人群中,小而密低密度脂蛋白与股骨粗隆间、全髋骨密度之间存在正相关,与腰椎骨密度之间无明显相关。按年龄分组后,<70岁人群中,小而密低密度脂蛋白与腰椎、髋部骨密度无明显相关;≥70岁人群中,小而密低密度脂蛋白与髋部所有部位均呈不同程度正相关,与腰椎骨密度之间无明显相关。偏相关分析结果显示,总人群中,小而密低密度脂蛋白与髋部骨密度之间的相关性消失;<70岁人群中,小而密低密度脂蛋白与骨密度之间仍无明显相关;≥70岁人群中,小而密低密度脂蛋白与股骨粗隆间、全髋骨密度之间仍存在正相关。小而密低密度脂蛋白与骨代谢指标之间的相关性结果示,≥70岁人群中,小而密低密度脂蛋白与Ⅰ型前胶原氨基端原肽、碱性磷酸酶之间呈负相关。结论 ≥70岁人群中小而密低密度脂蛋白与骨密度呈正相关,对于将小而密低密度脂蛋白作为老年性骨质疏松的一个潜在影响因素有一定的意义。Objective To investigate the correlation between small-dense low-density lipoprotein(sd-LDL)and bone mineral density(BMD)in patients with primary osteoporosis.Methods Using retrospective study,the study included 121 primary osteoporosis patients.BMD of the lumbar spine and the femoral neck was measured.Serum sd-LDL and bone metabolism index were collected.Simple linear regression analysis was performed among the three.Age and body weight were used as control variables to analyze the partial correlation between sd-LDL and BMD.Results In the total population,sd-LDL was positively correlated with intertrochanteric and hip BMD,but not with lumbar BMD.After grouping by age,there was no significant correlation between sd-LDL and BMD of the lumbar spine and hip in people less than 70 years old.sdLDL was positively correlated with all parts of the hip BMD in≥70 years of age group,but had no significant correlation with the lumbar BMD.Partial correlation analysis between sd-LDL and BMD showed that the association between sd-LDL and hip BMD disappeared in the total population.There was no significant correlation between sd-LDL and BMD in people<70 years of age group.There was still a positive correlation between sd-LDL and intertrochanteric and whole hip BMD in≥70 years of age group.The result of positive correlation between sd-LDL and bone metabolic biochemical markers showed that there was a negative correlation between sd-LDL and amino terminal propeptide of typeⅠprocollagen and alkaline phosphatase in people over 70 years old.Conclusion The correlation between sd-LDL and bone mineral density in osteoporotic population aged over 70 years has certain meaning for considering sd-LDL as a potential risk factor for senile osteoporosis.
