基于数据挖掘法探讨肖振卫教授治疗慢性肾脏病方药规律研究

摘　　要：目的:基于临床病例研究挖掘肖振卫教授治疗慢性肾脏病(Chronic Kidney Disease,CKD)的思路与用药规律。方法:收录肖振卫教授住院病例中符合CKD诊断标准的139首中药复方,建立肖振卫教授治疗CKD的中药数据库,运用SPSS 26.0软件进行药物频次统计和系统聚类分析,借助IBM SPSS Modeler 18.0软件对其进行数据挖掘分析,进行频次分析、中药功效归类分析、聚类分析、关联规则分析以及用药规律进行探究,进一步指导临床运用。结果:获得139首方剂,中药137味,总用药频次达到1827次,其中频次≥15的药物共26味,总的用药频次为1518次,占总用药频次的83%(1518/1827)。关联规则分析得出肖振卫教授治疗CKD的常用药对。聚类统计分析共得出6组药物序列。结论:肖振卫教授治疗CKD多以补肾健脾益气为本,药物多味甘性温,多从肝经最多,其次脾经、肾经等,注重中医整体观念,以肾为本的同时,重视脾胃功能的固护以及强调肝脏气机的重要意义。Objective:To explore the ideas and medication rules of Professor XIAO Zhenwei in the treatment of chronic kidney disease(CKD)based on clinical case studies.Methods:Among the hospitalized cases treated by Professor XIAO Zhenwei,a total Oof 139 TCM prescriptions in accordance with the diagnostic criteria of CKD were included,and the TCM database of Professor XIAO Zhenwei for the treatment of CKD was established.SPSS 26.0 software was used for drug frequency statistics and systematic cluster analysis,and IBM SPSS Modeler 18.0 software was used for data mining analysis.Frequency analysis,efficacy classification analysis of TCM medicine,cluster analysis,association rule analysis and medication law have been explored in order to further guide clinical application.Results:A total of 139 prescriptions and 137 TCM medicines were obtained,and the total frequency of medication reached 1827 times.Among them,there were 26 medicines with a frequency of≥15 times,and the total frequency of medication was 1518 times,accounting for 83%(1518/1827)of the total frequency of medication.Association rule analysis showed that Professor XIAO Zhenwei’s commonly used drug pairs in the treatment of CKD.A total of 6 groups of drug sequences were obtained by cluster statistical analysis.Conclusions:Professor XIAO Zhenwei’s treatment of CKD is mainly based on tonifying the kidney,strengthening the spleen and replenishing Qi(气).The drugs are mostly sweet and warm in nature.Most of them are from the liver meridian,followed by the spleen meridian and the kidney meridian.He pays attention to the overall concept of TCM.The preservation of function,and the importance of emphasizing the Qi mechanism of the liver.
