基于通调三焦理论治疗心力衰竭的古籍文献研究

摘　　要：近年来,中医药治疗心力衰竭有多环节、多方位、多靶点、多机制的优点,不仅可以显著改善临床症状,一定程度上可解决西医治疗中遇到的难题,如利尿剂抵抗,低心排心力衰竭的治疗等等,已经成为治疗心力衰竭的新亮点。文章通过对中医古籍文献的分析,探讨三焦的内涵、三焦的功能及三焦失调导致心力衰竭的病机。本研究共查阅古籍文献58部,涉及心力衰竭的病名、病因病机、症状、治则治法和预后等章节共56篇,三焦理论体系形成,不断认识、发展,与心力衰竭的关系等章节共29篇。三焦具有通行元气、运化水液和运行水谷的功能,参与全身水液代谢和水谷的转输过程,具有通行五脏六腑,气血经络的功能,若三焦功能失畅,脏腑功能失常,气血津液不能正常运化和敷布,痰饮水湿内停,气血经络壅滞,就会导致心力衰竭的出现。并进一步研究从通调三焦——开泻上焦、健运中焦、通利下焦治疗心力衰竭,具有深厚的经典理论基础及丰富的临床实践,充分体现了中医药在心力衰竭防治方面的优势。In recent years,the advantages of TCM in the treatment of heart failure with multiple links,multiple directions,multiple targets and multiple mechanisms can not only significantly improve clinical symptoms,but also solve the problems encountered in Western medicine treatment to a certain extent,such as diuretic resistance,treatment of heart failure with low cardiac output,etc.,which has become a new highlight in the treatment of heart failure.Through the analysis of ancient Chinese medicine texts,this paper discusses the pathogenesis of heart failure from the connotation of Sanjiao(三焦),the function of Sanjiao and Sanjiao disorder.In this study,a total of 58 ancient books were reviewed,involving 56 chapters on the name,etiology and pathogenesis,symptoms,treatment and prognosis of heart failure,and a total of 29 chapters on the formation of the Sanjiao theory system,continuous understanding and development,and the relationship with heart failure.Sanjiao has the function of passing Yuanqi(元气),transporting water and running nutrients,participating in the whole body water metabolism and nutrients transfer process,with the function of passing through the five internal organs,Qi(气)and blood meridians.If the three focal functions are not smooth,the internal organs are dysfunctional,Qi and blood can not be transported and compressed,with internal stagnation of fluid-dampness and blockage of meridians,which will lead to heart failure.And further research on the treatment of heart failure from dredging and regulating Sanjiao-discharging Shangjiao(上焦),running Zhongjiao(中焦),and open Xiajiao(下焦),which has a profound classical theoretical foundation and rich clinical practice,which fully reflects the advantages of TCM in the prevention and treatment of heart failure.
