华中3省公立医院医疗费用结构变动差异研究

摘　　要：目的:分析2009-2020年华中地区河南省、湖北省、湖南省公立医院医疗费用增长的结构变化情况,为建立科学合理的医疗费用控制机制提供参考。方法:收集2009-2020年河南省、湖北省、湖南省公立医院医疗费用相关统计数据,基于时间和空间维度,采用结构变动值、结构变动度、结构变动贡献率等指标进行比较分析。结果:湖北省、湖南省公立医院的门诊费用构成中,药费呈现明显下降趋势,河南省表现较为平稳。住院费用中,3省药费的结构变动值多表现为负值,但占比依然较大,检查费结构变动值均为正值。结论:华中3省公立医院的医疗费用结构需进一步优化,应通过规范公立医院收费行为、加强精细化运营管理、实现医疗服务价格动态调整等措施控制医疗费用不合理增长,促进优质医疗资源均衡布局,切实提升患者医疗福利水平。Objective:To analyze the structural changes of medical expenses growth in public hospitals in He'nan Province,Hubei Province and Hu'nan Province in Central China from 2009 to 2020,so as to provide a reference for establishing a scientific and reasonable medical expenses control mechanism.Methods:Collect statistical data related to medical expenses of public hospitals in He'nan,Hubei and Hu'nan provinces from 2009 to 2020,and conduct comparative analysis based on time and space dimensions,using indicators such as structural change value,structural change degree,and structural change contribution rate.Results:In the outpatient expenses of public hospitals in Hubei Province and Hu'nan Province,the drug expenses showed a significant downward trend,while the performance of He'nan Province was relatively stable.In the hospitalization expenses,the structural change values of drug expenses in the three provinces of Central China were mostly negative,but the proportion was still large,and the structural change values of examination expenses were all positive.Conclusion:The medical expenses structure of public hospitals in the three provinces of Central China needs to be further optimized.The unreasonable growth of medical expenses should be controlled by regulating the charging behavior of public hospitals,strengthening the fine operation and management,and realizing the dynamic adjustment of medical service prices,so as to promote the balanced distribution of high-quality medical resources and effectively improve the welfare level of patients.
