摘 要:目的比较心血管疾病人群与一般人群健康素养水平的差异,探讨居民健康素养水平与心血管疾病患病的关系。方法2020年9-11月,采用多阶段分层整群随机抽样的方法,在湖北省抽取12个县(市、区),使用《中国居民健康素养监测调查问卷》进行入户调查。依据是否患心血管疾病将调查对象分为心血管疾病人群和一般人群,对两类人群健康素养水平进行分析,使用logistic回归分析居民心血管疾病患病的影响因素。结果湖北省15~69岁心血管疾病人群健康素养水平为18.80%,一般人群在健康生活方式与行为素养、健康技能素养、传染病防治素养、慢性病防治素养和健康信息素养这5类素养水平(分别为29.43%,18.89%,28.87%,28.40%和30.03%)均高于心血管疾病人群(分别为23.46%,14.64%,21.96%,22.96%和23.79%),差异均有统计学意义(P均<0.05)。多因素logistic回归结果显示,具备健康信息素养为心血管疾病患病的保护因素(OR=0.737,95%CI:0.564~0.963,P<0.05)。结论心血管疾病患者健康素养水平偏低,且健康素养水平与心血管疾病患病状况存在一定关联,应加强健康行为与技能养成的干预。Objective To compare the levels of health literacy between cardiovascular disease patients and general residents,and to explore the relationship between health literacy and cardiovascular disease among residents.Methods By multi-stage stratified cluster random sampling method,residents in 12 counties/districts of Hubei province were extracted.Chinese Residents’Health Literacy Monitoring Questionnaire was used to conducted a household survey.The levels of health literacy of residents who reported cardiovascular diseases and not were compared,and a logistic regression was used to analysis the influencing factors of cardiovascular diseases.Results The level of health literacy among residents with cardiovascular diseases aged 15-69 was 18.80%.The general population had higher levels(29.43%,18.89%,28.87%,28.40%and 30.03%,respectively)than residents with cardiovascular disease(23.46%,14.64%,21.96%,22.96%and 23.79%,respectively)in five health literacy categories:health lifestyle and behavior,health skills,infectious disease prevention,chronic disease prevention and health information,with the statistical significant differences(all P were<0.05).The logistic regression analysis indicated that health information literacy was the protective factor of cardiovascular disease(OR=0.737,95%CI:0.564-0.963,P<0.05).Conclusions The residents with cardiovascular disease had lower level of health literacy,and there was a significant correlation between residents’health literacy and the susceptibility to cardiovascular disease.Interventions for developing health behaviors and health skills should be strengthened.
