摘 要:目的分析自发性高血压大鼠(SHR)不同心功能不全阶段左心室心肌的分层应变特征。方法28~102周龄的雄性SHR 54只,根据左心室射血分数(LVEF)与左心室舒张末压(LVEDP)分为心功能正常组(13只,周龄均为28周),舒张功能不全组(24只,周龄均在45~66周)和收缩功能不全组(17只,周龄均在84~102周),分别以28、45~66及84~102周Wistar-Kyoto(WKY)大鼠作为对照组,记录并比较各组大鼠的左心室结构指标、LVEF,以及在EchoPac上获得的左心室心肌各层纵向应变(LS)、周向应变(CS)及全层径向应变(RS)。采用Pearson相关分析SHR心肌应变指标与LVMI及LVEF的相关性。结果WKY大鼠的左心室心肌各层应变均无增龄性变化,且与SHR心功能正常组间差异无统计学意义。内层LS、内层CS和中层LS在SHR心功能正常组、舒张功能不全组、收缩功能不全组、WKY28周、WKY45~66周、和WKY84~102周之间比较,差异有统计学意义[内层LS分别为(-22.66±2.23)%比(-16.26±1.88)%比(-12.59±1.57)%比(-23.35±4.02)%比(-22.18±2.49)%比(-20.61±1.78)%,F=40.00,P<0.01;内层CS分别为(-24.78±1.59)%比(-21.83±3.27)%比(-13.22±1.55)%比(-25.90±4.48)%比(-24.83±2.91)%比(-25.23±3.65)%,F=35.53,P<0.01;中层LS分别为(-17.62±1.94)%比(-13.78±1.85)%比(-10.79±1.19)%比(-17.61±2.61)%比(-16.21±1.79)%比(-15.14±1.85)%,F=20.96,P<0.01]。SHR舒张功能不全组的内层LS、CS和中层LS低于心功能正常组;收缩功能不全组各向各层应变均低于心功能正常组和舒张功能不全组。SHR各向各层应变均与左室质量指数(LVMI)及LVEF相关,其中内层CS与LVMI的相关性r值最高(r=0.502,P<0.001),内层CS、中层CS、全层RS与LVEF的相关性r值较高(r值分别为-0.768、-0.785和0.759,均P<0.001)。结论SHR在舒张功能不全阶段只有内层LS、CS和中层LS降低,中、外层CS与全层RS受损才导致LVEF下降。Objective To analyze the characteristics of the left ventricular layer-specific strain of spontaneously hypertensive rats(SHR)in different stages of cardiac dysfunction.Methods Fifty-four male SHR aged 28 to 102 weeks were divided into three groups according to left ventricular ejection fraction(LVEF)and left ventricular end-diastolic pressure(LVEDP),including normal cardiac function group(n=13,28 weeks old),diastolic dysfunction group(n=24,45-66 weeks old)and systolic dysfunction group(n=17,84-102 weeks old).Wistar-Kyoto(WKY)rats of 28 weeks,45 to 66 weeks,and 84 to 102 weeks were used as the control groups.Morphological parameters of left ventricle,LVEF,as well as myocardial longitudinal strain(LS),circumferential strain(CS)of different layers and global radial strain(RS)obtained from EchoPac were recorded and compared in all groups.Pearson correlation analysis was used to analyze the correlation between mycardial strain index and LVMI,LVEF.Results There was no age-related change of left ventricular layer-specific strain in WKY rats,and no significant difference compared with SHR normal heart function group.There were statistically significant differences in inner-layer LS,inner-layer CS,and middle-layer LS among the groups of normal cardiac function,diastolic dysfunction,systolic dysfunction,WKY_(28 weeks),WKY_(45-66 weeks),and WKY_(84-102 weeks)[inner-layer LS:(-22.66±2.23)%vs(-16.26±1.88)%vs(-12.59±1.57)%vs(-23.35±4.02)%vs(-22.18±2.49)%vs(-20.61±1.78)%,F=40.00,P0.01;inner-layer CS:(-24.78±1.59)%vs(-21.83±3.27)%vs(-13.22±1.55)%vs(-25.90±4.48)%vs(-24.83±2.91)%vs(-25.23±3.65)%,F=35.53,P0.01;middle-layer LS:(-17.62±1.94)%vs(-13.78±1.85)%vs(-10.79±1.19)%vs(-17.61±2.61)%vs(-16.21±1.79)%vs(-15.14±1.85)%,F=20.96,P0.01].In SHR,the inner-layer LS,inner-layer CS and middle-layer LS were lower in the diastolic dysfunction group compared with the normal cardiac function group,and all layers of strain were significantly decreased in the systolic dysfunction group compared with the normal cardiac function
