摘 要:目的:探讨早产儿早发型感染外周血淋巴细胞亚群的表达及临床意义。方法:选取2019年8月—2021年1月汕头大学医学院第二附属医院新生儿科收治的140例早产儿,依据早产儿的感染情况分为早发型感染组(感染组,64例)与非感染组(76例),进行血常规、C反应蛋白及淋巴细胞亚群检测。结果:感染组胎膜早破37例(58.7%),高于非感染组[26例(41.3%)],差异有统计学意义(χ^(2)=7.82,P=0.005),其他一般资料差异均无统计学意义(P值均>0.05)。感染组外周血淋巴细胞计数为2.65(2.05,3.44)×10^(9)/L,低于非感染组[3.21(2.11,4.38)×10^(9)/L],差异有统计学意义(Z=-2.19,P=0.029)。感染组外周血CD4+CD8+T细胞百分比为0.71(0.46,1.07)%,高于非感染组[0.57(0.36,0.84)%],差异有统计学意义(Z=-2.19,P=0.029)。结论:早产儿早发型感染外周血淋巴细胞计数降低、CD4+CD8+T细胞百分比升高,可能对预测早产儿早发型感染具有一定临床价值。Objective:To investigate the expression and clinical significance of peripheral blood lymphocyte subsets in premature infants with early-onset infection.Methods:From August 2019 to January 2021,140 premature infants admitted to the Neonatology Department of the Second Affiliated Hospital of Shantou University Medical College were selected.According to the infection status of premature infants,they were divided into early-onset infection group(infection group,64 cases)and non-infection group(76 cases),blood routine,C-reactive protein and lymphocyte subsets were detected.Results:There were 37 cases(58.7%)of premature rupture of membranes in the infection group,which was higher than that in the non-infection group[26 cases(41.3%)],the difference was statistically significant(χ^(2)=7.82,P=0.005),and there was no statistically significant difference in other general data(all P>0.05).The peripheral blood lymphocyte count of the infected group was 2.65(2.05,3.44)×10^(9)/L,which was lower than that of the non-infected group[3.21(2.11,4.38)×10^(9)/L],and the difference was statistically significant(Z=-2.19,P=0.029).The percentage of CD4+CD8+T cells in the peripheral blood of the infection group was 0.71(0.46,1.07)%,which was higher than that of the non-infection group[0.57(0.36,0.84)%],and the difference was statistically significant(Z=-2.19,P=0.029).Conclusion:The decrease of peripheral blood lymphocyte count and the increase of CD4+CD8+T cells percentage in premature infants with early-onset infection may have certain clinical value in predicting early-onset infection in premature infants.
