摘　　要：血管性痴呆(Vascular Dementia,VaD)是一种以认知和记忆障碍为主要临床表现的痴呆类型,其发病主要因脑血管病变引起。该病为临床常见的中枢神经退行性疾病,是老年性痴呆的第二大病因,其发病率仅次于阿尔茨海默病,在中医学上属于“痴呆”“善忘”等范畴。随着社会人口结构不断老龄化和脑血管疾病的增加,该病的发病率呈指数式上升,严重危害人类的身心健康,也给家庭、社会和国家带来沉重的经济负担。不同于阿尔茨海默病,VaD为唯一可以防治的痴呆,近年来越来越多的研究将VaD作为脑血管疾病的关注重点。随着分子生物学技术的不断发展,微生物-肠-脑轴理论在抑郁症、脑卒中、痴呆等神经系统疾病中的作用受到广泛关注,越来越多的研究发现肠道内环境的紊乱与VaD的发生与发展密不可分,肠道菌群不仅可以通过神经递质、炎症反应等多种途径影响VaD的发生发展,还能够利用肠-脑轴的突触作用对中枢神经系统进行靶点调控,从而参与VaD的发病过程。近年来大量研究证实,中药介导的脑肠交互作用与VaD的发生发展关系密切;而其具体机制可能与中药单药及复方可调节肠道菌群、恢复肠稳态有关。中医扶阳学派弟子胡跃强教授善于运用“温肺降浊”的扶阳法论治VaD,并总结出其经验方“温肺降浊方”指导临床,疗效显著。文章基于此探讨温肺降浊方治疗VaD的作用机制,现介绍如下。Vascular dementia(VaD)is a type of dementia with cognitive and memory impairment as its main clinical manifestations,which is mainly caused by cerebrovascular lesions.As a common clinical central neurodegenerative disease,this disease is the second leading cause of senile dementia,and its incidence is second only to Alzheimer’s Disease.In traditional Chinese medicine,it belongs to the categories of dementia and amnesia.With the continuous aging of social population structure and the increase of cerebrovascular diseases,the incidence of the disease is rising exponentially,which seriously endangers the physical and mental health of human beings and also brings heavy economic burden to families,societies and countries.Different from Alzheimer’s Disease,vascular dementia is the only dementia that can be prevented and treated.In recent years,more and more studies focus on vascular dementia as a cerebrovascular disease.With the continuous development of molecular biology technology,the role of microbial-gut-brain axis theory in nervous system diseases such as depression,stroke and dementia has attracted wide attention.More and more studies have found that intestinal environmental disorders are closely related to the occurrence and development of vascular dementia.Intestinal flora can affect the occurrence and development of VaD through multiple pathways such as meurotransmitters inflammation,and target regulation of the central nervous system can be carried out through the gut-brain axis synaptic action,thus participating in the pathogenesis of VaD.In recent years,a large number of studies have confirmed that TCM medicine mediated brain-gut interaction is closely related to the occurrence and development of vascular dementia.The specific mechanism may be related to the regulation of intestinal flora and the restoration of intestinal homeostasis with TCM single medicinal and compound medicinals.Professor HU Yueqiang,a disciple of the Fuyang(扶阳)School of Traditional Chinese Medicine,is good at treating VaD by the
