摘　　要：血管性认知障碍(Vascular Cognitive Impairment,VCI)是由脑血管危险因素、明显或不明显的脑血管病导致的,临床表现多以记忆力减退、学习认知能力下降为主,严重影响患者及其家人的生活质量。中医对于VCI早有认识,虽无直接病名与之相对应,但以其临床特点记载论述。根据临床表现,其属于中医“呆证”“痴呆”范畴,病位在脑,涉及心、肝、脾、肾。患者脏腑功能衰退,气精失化,脑髓受损,阴阳失调,风、火、痰、瘀夹杂犯脑,致元神失养,神机失用而发病。病机特点为本虚标实,以肝肾精血亏损,气精失化为本,风、火、痰、瘀犯脑为标,病性为本虚标实,两者互为因果。治疗时应标本兼治,以补虚不忘调气化痰活血,祛邪不忘益精养血补气为原则。中医药学对于VCI的药物及非药物治疗的临床疗效有独特优势,文章从VCI的概念、流行病学、中医药学认识及治疗等方面对其研究进展进行阐述,以期临床工作者对VCI有更进一步的认识,进而有效改善VCI患者的记忆障碍,提高VCI患者认知功能康复及生活质量,进一步弘扬中医学“上工治未病”“未病先防,既病防变”,防患于未然的“治未病”理念,充分发挥中医药在防治VCI发生发展方面的优势,减少发病率,降低致残率,更大程度地减轻患者的痛苦,减轻患者家庭和社会的负担。Vascular cognitive impairment(VCI)is caused by cerebrovascular risk factors and obvious or insignificant cerebrovascular diseases.The clinical manifestations are mainly memory loss and learning and cognitive decline,which seriously affect the quality of life of patients and their families.Traditional Chinese medicine has recognized VCI for a long time,although there is no direct disease name,it is described according to its clinical characteristics.According to its clinical manifestations,it belongs to the category of“stupidity syndrome”and“dementia”in traditional Chinese medicine.The disease location is in the brain,involving the heart,liver,spleen and kidney.The patient’s viscera function declines,Qi(气)and essence are lost,brain marrow is damaged,Yin(阴)and Yang(阳)are out of balance,wind,fire,phlegm and blood stasis invade the brain,resulting in the loss of vitality and the loss of normal mechanism.The pathogenesis features are difficiency as the root cause and excess as the symptom.The loss of liver and kidney essence and blood,the loss of Qi and essence is the root cause while the wind,fire,phlegm and blood stasis invading the brain is the symptom,and the disease nature is the intermingled deficiency and excess,the two are mutually cause and effect.The treatment should be based on the principle of tonifying deficiency in addition to regulating Qi,removing phlegm and promoting blood circulation;nourishing blood and Qi,and expelling pathogenic factors,which targets both the root causes and the symptoms.Traditional Chinese medicine has unique advantages in the clinical efficacy of drug and non drug treatment of VCI.The paper expounds the research progress of VCI from the perspectives of its concept,epidemiology,understanding and TCM treatment,so as to have a further understanding of VCI for clinical workers,effectively relieve the memory disorder of VCI patients,and improve the cognitive function rehabilitation and life quality of VCI patients.It is to further promote the TCM concept of“curing d
