曾朝辉自拟腰痛方治疗腰椎间盘突出症(寒湿阻络证)的经验

摘　　要：文章介绍了曾朝辉自拟腰痛方治疗腰椎间盘突出症(寒湿阻络证)的经验。腰椎间盘突出症是脊柱外科最常见的临床慢性退行性疾病,中医上属“腰痛”范畴,腰椎间盘突出症中医辨证分型众多,其中寒湿阻络型为常见的证型,在中国因大部分地区秋冬天的寒冷,加之患者自身身体的劳累,使寒湿之邪更容易侵袭腰部,进而气血凝滞于腰部,从而慢慢出现腰部疼痛等一系列临床症状。曾教授具有扎实的中医理论和现代医学理论知识基础,临床经验丰富,擅长从先天之本角度分析脏腑的盛衰在腰痛病发病过程的变化及其作用。腰椎间盘突出症中,曾教授主张抓住“本虚标实”的病因病机,由肾虚内因入手,兼顾寒湿外因,针对寒湿阻络型腰椎间盘突出症,以补肾散寒祛湿、温经通络为法,方用自拟腰痛方治疗腰椎间盘突出症(寒湿阻络证),其主要由独活、白芷、桃仁、延胡索、牛膝、杜仲、续断、丹参、甘草等药物组成,实际根据患者临床症状进行加减。经大量临床实践证明,该方临床疗效显著,对于临床治疗腰椎间盘突出症(寒湿阻络证)有很好的指导意义。This paper introduces ZENG Zhaohui’s experience in treating lumbar intervertebral disc herniation(syndrome of cold dampness blocking collaterals)with self-designed Yaotong formula(腰痛方).Lumbar intervertebral disc herniation is the most common clinical chronic degenerative disease in spinal surgery,which belongs to the category of“lumbago”in traditional Chinese medicine.There are many syndrome types of lumbar intervertebral disc herniation in traditional Chinese medicine,among which syndrome of cold dampness blocking collaterals is the common syndrome type.In China,due to the cold and dampness in most regions in autumn and winter,combined with the fatigue of the body,the evil of cold and dampness is more likely to attack the waist,and then the waist suffers from stagnation of Qi(气)and blood,which will gradually lead to a series of clinical symptoms such as lower back pain.Professor ZENG has a solid theoretical knowledge foundation of traditional Chinese medicine and modern medical theory,and rich clinical experience.He is good at analyzing the changes and effects of the rise and fall of viscera in the pathogenesis of lumbago disease from the perspective of congenital basis.In the case of lumbar intervertebral disc herniation,Professor ZENG advocates that the etiology and pathogenesis of“asthenia in origin and asthenia in superficiality”should be grasped and starting from the internal causes of kidney deficiency and taking into account the external causes of cold and dampness.For the lumbar intervertebral disc herniation with syndrome of cold and dampness blocking collaterals,the method of tonifying the kidney to dissipate cold and dampness and warming the meridian to remove collaterals should be taken.Self-designed Yaotong formula mainly consists of Duhuo(Radix Angelicae Tuhuo),Baizhi(Angelica Dahurica),Taoren(Peach seeds),Yanhusuo(Rhizoma corydalis),Niuxi(Rhizoma bidentalis),Duzhong(Eucommia ulmoides),Xuduan(Conjunctivitis),Danshen(Salvia miltiorrhiza),and Gancao(Licorice),etc.,for the treatment
