摘 要:目的基于1.5 T磁共振T2定量成像预测乳腺癌微卫星不稳定性的临床价值。方法回顾性选取2018年5月至2021年5月重庆医科大学附属巴南医院收治的56例乳腺癌患者作为研究对象,根据患者乳腺癌微卫星稳定性分为稳定性组(n=39)和不稳定性组(n=17)。患者均接受1.5 T磁共振T2定量成像检查。对比两组患者T2定量成像参数(T2值、离散系数)以及临床资料,分析影响乳腺癌患者微卫星不稳定性的危险因素,并绘制ROC曲线分析基于1.5 T磁共振T2定量成像预测乳腺癌微卫星不稳定性的临床价值。结果不稳定性组患者T2值和离散系数为96.43±5.78、0.67±0.12,显著高于稳定性组患者(81.27±5.29、0.32±0.09),差异均有统计学意义(P<0.05)。不稳定组患者存在腋窝淋巴结转移和临床分期Ⅲ期占比为76.47%、58.82%,显著高于稳定性组患者(46.15%、2.56%),差异均有统计学意义(P<0.05)。T2值、离散系数、存在腋窝淋巴结转移和临床分期Ⅲ期均是乳腺癌患者微卫星不稳定性的影响因素;ROC曲线结果提示:T2值、离散系数对于患者微卫星不稳定性的预测敏感度分别为76.5%、82.4%,特异度分别为64.1%、74.4%,两者联合检测预测敏感度为88.2%,特异度为89.7%,显著高于各项指标单独检测,差异均有统计学意义(P<0.05)。结论基于1.5 T磁共振T2定量成像参数T2值和离散系数联合预测乳腺癌患者微卫星不稳定性的敏感度和特异度较高,且T2值和离散系数是乳腺癌患者微卫星不稳定性的危险因素。Objective To evaluate the clinical value of T2-mapping sequences at 1.5T magnetic resonance imaging for predicting microsatellite instability in breast cancer.Methods Fifty-six patients with breast cancer in Banan Hospital Affiliated to Chongqing Medical University from May 2018 to May 2021 were enrolled,and classified into stability group(n=39)and instability group(n=17)according to the microsatellite stability in breast cancer patients.All patients underwent T2-mapping sequence scans at 1.5T magnetic resonance imaging.The T2-mapping parameters(T2 values,dispersion coefficients)as well as the clinical data were compared between two groups.Receiver Operating Characteristic(ROC)curve was plotted to evaluate the clinical value of T2-mapping sequences at 1.5T magnetic resonance imaging for predicting microsatellite instability in breast cancer.Results The T2 value and dispersion coefficient of patients in the unstable group were 96.43±5.78 and 0.67±0.12,which were significantly higher than those in the stable group(81.27±5.29 and 0.32±0.09),and the differences were statistically significant(P<0.05).The proportion of patients with axillary lymph node metastases and clinical stageⅢin instability group were 76.47%,58.82%,which were significantly higher those of stability group(46.15%,2.56%),the differences were statistically significant(P<0.05).T2 values,dispersion coefficients,axillary lymph node metastases and clinical stageⅢwere all factors affecting microsatellite instability in breast cancer patients.ROC curve results suggested that the sensitivity of T2 value and dispersion coefficient for predicting microsatellite instability in patients was 76.5%and 82.4%,and the specificity was 64.1%and 74.4%,respectively.The combined test had a predictive sensitivity of 88.2%and a specificity of 89.7%,which were significantly higher than those of separate test(P<0.05).Conclusion The T2-mapping sequences at 1.5T parameters T2 values and dispersion coefficients obtain high sensitivity and specificity in prediction of mi
