鼻内镜改良鼻中隔矫正术治疗创伤性歪鼻伴鼻中隔偏曲患者的效果

摘　　要：目的:观察鼻内镜改良鼻中隔矫正术治疗创伤性歪鼻伴鼻中隔偏曲患者的效果。方法:选取2018年7月至2020年8月该院收治的90例创伤性歪鼻伴鼻中隔偏曲患者进行前瞻性研究,根据随机数字表法将其分为研究组和对照组各45例。研究组行鼻内镜改良鼻中隔矫正术治疗,对照组行开放鼻中隔黏膜下切除术治疗,比较两组临床疗效、临床相关指标(手术时间、鼻外观复常时间、住院时间)水平,术前、术后6个月外鼻测量值(a、b、c、d、e、f、g线),以及术后1个月并发症发生率。结果:研究组治疗总有效率为93.33%(42/45),高于对照组的71.11%(32/45),差异有统计学意义(P<0.05);研究组手术时间、鼻外观复常时间和住院时间均短于对照组,差异有统计学意义(P<0.05);术后,研究组d、f线均小于术前,且小于对照组,e、g线均大于术前,且大于对照组,差异有统计学意义(P<0.05);对照组手术前后a、b、c、d、e、f、g线比较,差异均无统计学意义(P>0.05);研究组并发症发生率为2.22%(1/45),低于对照组的22.22%(10/45),差异有统计学意义(P<0.05)。结论:鼻内镜改良鼻中隔矫正术治疗创伤性歪鼻伴鼻中隔偏曲患者可提高治疗总有效率,改善d、e、f、g线,降低并发症发生率,效果优于开放鼻中隔黏膜下切除术治疗。Objective:To observe effects of modified nasal endoscopic nasal septum correction in treatment of traumatic twisted nose with nasal septum deviation.Methods:A prospective study was conducted on 90 patients with traumatic twisted nose and nasal septum deviation admitted to the hospital from July 2018 to August 2020.They were divided into study group and control group according to the random number table method,45 cases in each.The study group was treated with modified nasal endoscopic nasal septum correction,while the control group was treated with open nasal septum submucosal resection.The clinical efficacy,the clinical related indicator levels(operation time,nasal appearance recovery time,hospitalization time),the external nasal measurements(a,b,c,d,e,f,g lines)before and 6 months after the operation,and the incidence of complications 1 month after the operation were compared between the two groups.Results:The total effective rate of treatment in the study group was 93.33%(42/45),which was higher than 71.11%(32/45)in the control group,and the difference was statistically significant(P<0.05).The operation time,the nasal appearance recovery time and the hospitalization time of the study group were shorter than those of the control group,and the differences were statistically significant(P<0.05).After the operation,the d and f lines in the study group were smaller than those before the operation and smaller than those in the control group;the e and g lines were larger than those before the operation and larger than those in the control group;and the difference was statistically significant(P<0.05).There were no significant differences in a,b,c,d,e,f and g lines before and after the operation in the control group(P>0.05).The incidence of complications in the study group was 2.22%(1/45),which was lower than 22.22%(10/45)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Modified endoscopic nasal septum correction can improve the total effective rate of treatment,improve the d,e,
