脱细胞基质生物补片与聚丙烯补片在男性腹股沟疝患者无张力疝修补术中的应用效果比较

摘　　要：目的:比较脱细胞基质生物补片与聚丙烯补片在男性腹股沟疝患者无张力疝修补术中的应用效果。方法:回顾性分析2018年10月至2021年10月于该院行无张力疝修补术的120例男性腹股沟疝患者的临床资料,根据术中所用补片不同分为对照组和观察组各60例。对照组采用聚丙烯补片,观察组采用脱细胞基质生物补片,比较两组手术相关指标(手术时间、术中出血量、住院时间)水平、术后疼痛[视觉模拟评分法(VAS)]评分、手术前后精液质量(精液量、精子密度、a+b级精子百分率)和并发症发生率。结果:两组手术时间、术中出血量比较,差异均无统计学意义(P>0.05);观察组住院时间短于对照组,差异有统计学意义(P<0.05);术后24、48 h,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);术后,两组精液量、精子密度、a+b级精子百分率均低于术前,但观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为5.00%(3/60),低于对照组的20.00%(12/60),差异有统计学意义(P<0.05)。结论:与聚丙烯补片相比,脱细胞基质生物补片用于男性腹股沟疝患者无张力疝修补术中可缩短住院时间,减轻术后疼痛及对睾丸和输精管组织的影响,降低术后并发症发生率。Objective:To compare effects of acellular matrix biological mesh and polypropylene mesh in male patients with inguinal hernia undergoing tension-free hernia repair.Methods:The clinical data of 120 male patients with inguinal hernia who underwent tension-free repair in this hospital from October 2018 to October 2021 were retrospectively analyzed.According to the different patches used during the operation,they were divided into control group and observation group,60 cases in each.The control group was treated with polypropylene patch,while the observation group was treated with acellular matrix biological patch.The operation related index levels(operation time,intraoperative blood loss,hospitalization time),the postoperative pain[visual analogue scale(VAS)]score,the semen quality before and after the operation(semen volume,sperm density,a+b grade sperm percentage),and the incidence of complications were compared between the two groups.Results:There were no significant differences in the operation time and the intraoperative blood loss between the two groups(P>0.05).The hospitalization time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).24 and 48 h after the operation,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After the operation,the semen volume,the sperm density and the a+b grade sperm percentage in the two groups were lower than those before the operation,but those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 5.00%(3/60),which was lower than 20.00%(12/60)in the control group,and the difference was statistically significant(P<0.05).Conclusions:The acellular matrix biological patch in the male patients with inguinal hernia undergoing tension-free hernia repair can shorten the hospitalization
