活血祛瘀汤联合手法复位石膏外固定治疗桡尺骨干双骨折患者的效果

摘　　要：目的:观察活血祛瘀汤联合手法复位石膏外固定治疗桡尺骨干双骨折患者的效果。方法:选取2020年7月至2022年7月该院收治的60例桡尺骨干双骨折患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各30例。对照组采用手法复位石膏外固定联合常规西药治疗,观察组采用手法复位夹石膏固定联合活血祛瘀汤治疗。比较两组疼痛程度[视觉模拟评分法(VAS)]评分、疼痛有效缓解率、肿胀有效缓解率、治疗前后血液流变学指标[全血黏度、红细胞比容、纤维蛋白原]水平和血清炎性因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)]水平。结果:治疗后3、5 d,两组VAS评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组疼痛有效缓解率、肿胀有效缓解率均明显高于对照组,差异有统计学意义(P<0.05);治疗后,两组全血黏度、红细胞比容、纤维蛋白原、hs-CRP、TNF-α、IL-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:活血祛瘀汤联合手法复位石膏外固定治疗桡尺骨干双骨折患者可提高疼痛和肿胀有效缓解率,降低VAS评分、炎性因子水平和血液流变学指标水平,优于常规西药联合手法复位石膏外固定治疗效果。Objective:To observe effects of Huoxue Quyu decoction combined with manual reduction and splint external fixation in treatment of patients with radial and ulnar shaft fractures.Methods:A prospective study was conducted on 60 patients with radial and ulnar shaft fractures admitted to this hospital from July 2020 to July 2022.They were divided into control group and observation group according to the random number table method,30 cases in each.The control group was treated with manual reduction and plaster external fixation combined with conventional western medicine,while the observation group was treated with manual reduction and splint fixation combined with Huoxue Quyu decoction.The pain degree score,the effective pain relief rate,the effective swelling relief rate,the hemorheology index levels(whole blood viscosity,hematocrit,fibrinogen),and the serum inflammatory factor levels[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]were compared between the two groups before and after the treatment.Results:3 d and 5 d after the treatment,the VAS scores of the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The effective remission rate of pain and swelling in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).Further,after the treatment,the levels of whole blood viscosity,hematocrit,fibrinogen,hs-CRP,TNF-αand IL-6 in the two groups were lower than those before the treatment,those in the observation group were those in lower than the control group,and the differences were statistically significant(P<0.05).Conclusions:Huoxue Quyu decoction combined with manual reduction and splint external fixation in the treatment of the patients with radial and ulnar shaft fractures can improve the effective relief rate of pain and swelling,and reduce the VAS sc
