摘 要:目的:比较256层螺旋CT血管造影(256-SCTA)与磁共振血管成像(MRA)诊断颅内动脉瘤的价值。方法:选取2018年1月至2022年3月该院收治的206例疑似颅内动脉瘤患者进行前瞻性研究,均行数字减影血管造影(DSA)、256-SCTA、MRA检查,以DSA检查结果为金标准,比较256-SCTA、MRA诊断颅内动脉瘤的效能(灵敏度、特异度、准确度、误诊率、漏诊率),并比较256-SCTA、MRA诊断颅内动脉瘤的瘤颈、瘤体最大径。结果:DSA检查结果显示,206例疑似颅内动脉瘤患者中阳性146例,阴性60例;256-SCTA诊断颅内动脉瘤的灵敏度、准确度均高于MRA,漏诊率低于MRA,差异有统计学意义(P<0.05);256-SCTA、MRA诊断颅内动脉瘤的特异度、误诊率比较,差异无统计学意义(P>0.05);256-SCTA、MRA诊断颅内动脉瘤的瘤颈、瘤体最大径比较,差异均无统计学意义(P>0.05)。结论:256-SCTA诊断颅内动脉瘤的灵敏度、准确度高于MRA,漏诊率低于MRA。Objective:To compare value of 256-slice spiral CT angiography(256-SCTA)and magnetic resonance angiography(MRA)in diagnosis of intracranial aneurysms.Methods:A prospective study was conducted on 206 patients with suspected intracranial aneurysms from January 2018 to March 2022.All patients underwent digital subtraction angiography(DSA),256-SCTA and MRA.Using the results of DSA as the gold standard,the efficacies of 256-SCTA and MRA in the diagnosis of intracranial aneurysms(sensitivity,specificity,accuracy,misdiagnosis rate,missed diagnosis rate)were compared.Further,the levels of aneurysm neck and the maximum diameters of intracranial aneurysms diagnosed by 256-SCTA and MRA were compared.Results:Among the 206 suspected intracranial aneurysm patients,DSA examination diagnosed 146 cases as positive and 60 cases as negative for intracranial aneurysm.The sensitivity and the accuracy of 256-SCTA examination in diagnosing intracranial aneurysms were higher than MRA examination,the missed diagnosis rate was lower than MRA examination,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the specificity and the misdiagnosis rate between 256-SCTA and MRA in the diagnosis of intracranial aneurysms(P>0.05).There were no statistically significant differences in the neck and the maximum diameter of intracranial aneurysms diagnosed by 256-SCTA and MRA examinations(P>0.05).Conclusions:The sensitivity and the accuracy of 256-SCTA in the diagnosis of intracranial aneurysms are higher than those of MRA,and the missed diagnosis rate is lower than that of MRA.
