摘 要:目的分析临床送检标本分离出的真菌菌种分布及其耐药情况,为临床合理用药提供依据。方法收集2019年1月至2021年12月首都医科大学附属北京妇产医院临床标本培养出的真菌616株,使用法国生物梅里埃VITEK 2 COMPACT自动分析仪、VITEKMS质谱鉴定仪及ATB GUNGUS 3真菌药敏试剂盒进行鉴定和药敏试验。结果共检出616株真菌,其中白色念珠菌、光滑念珠菌、近平滑念珠菌、克柔念珠菌的检出率分别为75.16%(463/616)、15.26%(94/616)、3.41%(21/616)、2.11%(13/616)。616株真菌对伊曲康唑敏感率最低(45.78%),其次为氟康唑(69.32%)、伏立康唑(73.05%)、5-氟胞嘧啶(95.29%)、两性霉素(100%)。白色念珠菌、光滑念珠菌、近平滑念珠菌对氟康唑的敏感率分别为69.33%、74.47%、90.48%,近平滑念珠菌对5种药物敏感率最高(90.48%~100%)。白色念珠菌、光滑念珠菌、近平滑念珠菌、克柔念珠菌分别对5-氟胞嘧啶、伊曲康唑敏感率差异有统计学意义(P<0.0001)。结论检出的真菌以白色念珠菌为主,不同种类真菌对不同药物的敏感性有差异,应根据药敏结果合理用药。Objective to analyze the distribution and drug resistance of fungi isolated from clinical specimens in Beijing Obstetrics and Gynecology Hospital,and to provide basis for clinical rational drug use.Methods 616 strains of fungi cultured from clinical specimens of Beijing Obstetrics and Gynecology Hospital affiliated to Capital Medical University from January 2019 to December 2021 were collected,and identified and tested by VITEK 2 COMPACT automatic analyzer,VITEK MS mass spectrometer and ATB GUNGUS 3 fungus drug sensitivity kit.Results A total of 616 strains of fungi were detected,of which the detectable rates of Candida albicans,Candida glabra,Candida subleucoides and Candida clenosus were 75.16%(463/616),15.26%(94/616),3.41%(21/616)and 2.11%(13/616),respectively.The susceptibility rate of 616 fungi to itraconazole was the lowest(45.78%),followed by fluconazole(69.32%),voriconazole(73.05%),5-fluorocytosine(95.29%)and amphotericin(100%).The susceptibility rates of Candida albicans,Candida glabra and Candida paraphylaxis to fluconazole were 69.33%,74.47%and 90.48%,respectively,and Candida paraphylaxis had the highest susceptibility rate to five drugs(90.48%~100%).The susceptibility rates of Candida albicans,Candida glabra,Candida subflora to 5-fluorocytosine and itraconazole were significantly different(P<O.0001).Conclusion Candida albicans is the main fungus detected in Beijing Obstetrics and Gynecology Hospital,and the sensitivity of different fungi to different drugs is different,It is necessary to use drugs rationally according to the results of drug sensitivity in clinic to improve the clinical treatment effect.
