摘 要:目的检测过敏性鼻炎(AR)患者外周血辅助T细胞9(Th9)、辅助T细胞22(Th22)、调节性T细胞(Treg)的比例及血清中白细胞介素-9(IL-9)、白细胞介素-22(IL-22)、转化生长因子β(TGF-β)和总IgE的表达水平,讨论其与过敏性鼻炎的关系以及临床意义。方法选择48例过敏性鼻炎患者为患者组,40例健康成人作为对照组,采用流式细胞术检测外周血单个核细胞(PBMCs)中Th9、Th22和Treg细胞的比例;用酶联免疫吸附法(ELISA)检测血清中细胞因子IL-9、IL-22和TGF-β的含量;用速率散射比浊法检测血清中总IgE的水平。测得数据用SPSS 24.0统计软件进行分析,用Pearson相关分析进行相关性检验。结果同对照组相比,AR患者组Th9细胞的百分比显著升高(P<0.0001),Treg细胞的百分比显著降低(P<0.0001),Th22细胞的百分比与健康对照组相比无显著差异(P=0.63)。与对照组相比,AR患者组血清IL-9、IL-22和总IgE表达水平显著升高(P<0.0001),而TGF-β则呈现明显下降(P<0.0001)。AR患者血清IL-9与总IgE水平呈正相关(r=0.487,P<0.05),TGF-β与总IgE水平呈负相关(r=-0.679,P<0.05)。结论外周血Th9、Treg细胞及细胞因子的分泌异常、比例失调在过敏性鼻炎的发病过程中起着重要作用,可作为病情诊断的重要参考指标,也可为临床病因的分析研究提供理论依据,而Th22细胞目前不明确,可能没有参与AR的发病机制。Objective To examine the proportion of T helper 9(Th9),T helper 22(Th22),regulatory T(Treg)cells and the expression levels of interleukin-9(IL-9),interleukin-22(IL-22),TGF-β(transforming growth factor-β)and total IgE in peripheral blood of allergic rhinitis(AR)patients,and to investigate their correlation and significance in AR.Methods A total of 88adult subjects were selected,including AR group(n=48)and healthy control group(n=40).Flow cytometry was used to test the percentages of Th9,Th22and Treg cells in the peripheral blood mononuclear cells(PBMCs).Enzyme linked immunosorbent assay(ELISA)was used to detect the expression level of IL-9,IL-22and TGF-β.The content of total IgE in serum was measured by rate nephelometry.The data were analyzed by SPSS 24.0 statistical software.The correlation test was conducted by Pearson linear correlation analysis.Results Compared with the healthy control group,the percentage of Th9cells in the peripheral blood mononuclear cells was significantly increase in AR group(P<0.0001).On the contrary,the ratio of Treg cells was declined markedly(P<0.0001),There was no significant between healthy controls and AR(P=0.63).Compared with the control group,in the serum of AR patients,the expression levels of IL-9,IL-22and IgE were obviously elevated,the level of TGF-βdescended visibly(P<0.0001).The expression of IL-9in serum of AR patients displayed a kind of positive correlation with total IgE expression(r=0.487,P<0.05),the level of TGF-βwas negatively correlated with total IgE level(r=-0.679,P<0.05).Conclusion Th9,Treg cells and their relevant cytokines play an important role in the pathogenesis of allergic rhinitis.It can be used as an important reference index for disease diagnosis,and it can also provide a theoretical basis for the analysis and research of clinical etiology.while Th22cells are currently unclear and may not be involved in the pathogenesis of AR.
