摘 要:目的旨在分析血清脂蛋白a[Lp(a)]在中青年急性心肌梗死(AMI)预测中的价值和作用,并探讨其预测因素的独立性。方法选取长春市中心医院2021年10月至2023年3月收治的年龄在20-60周岁AMI患者60例为研究组,同期同年龄段行冠状动脉造影检查(CAG)且Gensini评分为1分以下患者60例为对照组,采用免疫透视比浊法测定患者的血清脂蛋白a水平,采用Gensini评分法评估冠状动脉狭窄程度。结果AMI患者的Lp(a)水平明显高于对照组(P<0.05),且预测的AUC值为0.713,表明Lp(a)水平具有较高预测价值。最佳Lp(a)水平截断值为0.393,此时其特异度为0.590,敏感度为0.803。结论Lp(a)可作为中青年AMI的预测因子,且预测中青年AMI的发生具有很高的独立性和预测价值。Objective To analyze whether serum lipoprotein a level can be used as a predictor of acute myocardial infarction in young and middle-aged people and its predictive value.Methods A total of 60AMI patients,aged 20-60 years,who were admitted to Changchun Central Hospital hospital from October to march in the 20212023,were divided into three 1groups according to the severity of coronary artery disease:Group A,Group B and Group C,meanwhile,60 patients with non-acute myocardial infarction(Gensini score less than 1)were selected as control group.The levels of serum lipoprotein a were measured in the study group and the control group,and the differences of serum lipoprotein a levels were compared among the groups.Results The level of LP(a)was higher than that in control group(135.27±6.02)mg/L,the difference was statistically significant(P<0.05).The ROC curve showed that LP(a)level corresponded to an AUC of 0.713,implying a higher diagnostic value for the disease,and corresponded to an optimal cut-off of 0.393.Conclusion LP(a)can be used as a predictive factor to predict the occurrence of AMI in young and middle-aged people,and it has high predictive value.
