摘 要:目的研究宫颈高危型HPV感染者外周血T细胞亚群及其免疫因子变化及其临床意义。方法收集宫颈高危型HPV-DNA阳性148例为病例组,其中CINⅠ41例,CINⅡ38例,CINⅢ38例,宫颈鳞癌(SCC)31例;同时选取高危型HPV-DNA阴性健康者40例为对照,检测各组外周血T细胞亚群及血清中免疫因子含量,分析各组之间差异。结果与对照组相比,随着宫颈病变进展,CINⅠ、CINⅡ、CINⅢ及SCC组外周血CD4^(+)T细胞比例含量逐渐减低,CD8^(+)T细胞比例逐渐升高,CD4^(+)/CD8^(+)比例逐渐减低,差异有统计学意义(P<0.05)。HP 16/18型与其他型别感染组相比,血清IFN-γ含量逐渐减低,IL-4、IL-17及TGF-β1含量逐渐升高,差异有统计学意义(P<0.05)。结论宫颈高危型HPV感染伴宫颈病变患者及宫颈鳞癌患者体内存在不同程度的细胞免疫减低,检测T细胞亚群及其免疫因子变化可用于进一步研究宫颈高危型HPV感染的免疫机制,为临床高危型HPV感染伴宫颈病变免疫干预及预后提供客观依据。Objective To analyse the changes of T cell subsets and immune factors and its clinical significance in peripheral blood of patients with high-risk HPV infection in the cervix.Methods 148cases with high-risk HPV-DNA positive were collected including 41cases of CINⅠ,38cases of CINⅡ,38cases of CINⅢand 31cases of squamous cell carcinoma(SCC);Meanwhile,40cases with high-risk HPV-DNA negative of healthy people were selected as the control group.Detecting the level of the T cell subsets and immune factors in peripheral and the differences were analysed among the groups.b Results Compared with the control group,with the progress of the pathological staging,the level of CD4^(+)T cells were decreased gradually in groups of CINⅠ,CINⅡ,CINⅢand SCC;CD8^(+)T cells were increased gradually,the ratio of CD4^(+)/CD8^(+)were decreased gradually,the differences were significant(P<0.05).The trend of the changes for immune factors with hish-risk HPV infection in other types was consistent with HPV16/18.Compared with the control group,the content of IFN-γwere decreased gradually,IL-4,IL-17and TGF-β1were increased gradually among the four groups,the differences were significant(P<0.05).Conclusion Cell-medicated immunity decreased in patients with high-risk HPV infection for high grade cervical lesions and squamous cell carcinoma of the cervix,detecting the changes of T cell subsets and immune factors can be used to further study the immune mechanism of high-risk HPV infection of cervix,and provide objective basis for the immune intervention and prognosis of clinical high-risk HPV infection.
