血清降钙素原、白细胞介素-6、C反应蛋白及白细胞计数检测在呼吸道感染诊断中的临床价值评价

摘　　要：目的 分析血清降钙素原(PCT)、白细胞介素-6(IL-6)、C反应蛋白(CRP)及白细胞计数(WBC)检测在呼吸道感染诊断中的临床价值。方法 选择200例呼吸道感染患者,将100例细菌感染患者设为A组,100例非细菌感染者设为B组;另选择100例健康体检者作为对照组。比较三组PCT、IL-6、CPR、WBC水平;A组和B组血清PCT、IL-6、CPR、WBC单独及联合检测的阳性检出率。结果 A组血清PCT、WBC、IL-6、CPR水平分别为(1.87±0.22)μg/L、(12.27±2.16)×10^(9)/L、(38.32±4.96)pg/ml、(13.32±3.10)mg/L,B组分别为(0.74±0.10)μg/L、(9.18±1.89)×10^(9)/L、(26.04±2.12)pg/ml、(11.14±2.74)mg/L,对照组分别为(0.30±0.02)μg/L、(6.22±1.20)×10^(9)/L、(7.47±2.41)pg/ml、(7.12±1.01)mg/L。三组血清PCT、WBC、IL-6、CPR水平比较差异具有统计学意义(P<0.05);A组PCT、WBC、IL-6、CPR水平均显著高于B组和对照组,B组高于对照组,差异具有统计学意义(P<0.05)。A组血清PCT、IL-6、CPR、WBC单独及联合检测的阳性检出率分别为96.0%、76.0%、84.0%、93.0%、100.0%,B组分别为16.0%、22.0%、62.0%、27.0%、47.0%。两组PCT+WBC+IL-6+CRP检测的阳性检出率均高于本组血清PCT、IL-6、CPR、WBC单独检测,差异具有统计学意义(P<0.05);A组血清PCT、IL-6、CPR、WBC单独及联合检测的阳性检出率均高于B组,差异具有统计学意义(P<0.05)。结论 对呼吸道感染患者单独应用PCT、CRP、IL-6、WBC检测均可获得相对准确的诊断结果,但联合检测的诊断价值更高,更有助于提升诊断准确率,对是否发生细菌感染进行及时有效的鉴别,为后续更好地制定治疗计划提供客观、科学的理论依据。Objective To analyze the clinical value of serum procalcitonin(PCT),interleukin-6(IL-6),C-reactive protein(CRP)and white blood cell count(WBC)in the diagnosis of respiratory tract infection.Methods 200 patients with respiratory tract infection,of which 100 patients with bacterial infection were selected as group A and 100 patients with non-bacterial infection were selected as group B.Another 100 patients who received physical examination during the same period were selected as control group.The serum levels of PCT,IL-6,CPR and WBC were compared among the three groups,as well as the positive detection rate of serum PCT,IL-6,CPR and WBC alone and in combination in group A and group B.Results Serum levels of PCT,WBC,IL-6 and CPR were(1.87±0.22)μg/L,(12.27±2.16)×10^(9)/L,(38.32±4.96)pg/ml and(13.32±3.10)mg/L in group A,(0.74±0.10)μg/L,(9.18±1.89)×10^(9)/L,(26.04±2.12)pg/ml and(11.14±2.74)mg/L in group B,and(0.30±0.02)μg/L,(6.22±1.20)×10^(9)/L,(7.47±2.41)pg/ml and(7.12±1.01)mg/L in the control group.Serum levels of PCT,WBC,IL-6 and CPR were compared among the three groups,and the difference was statistically significant(P<0.05).Serum levels of PCT,WBC,IL-6 and CPR in group A were significantly higher than those in group B and control group,and group B was higher than control group.The differences were statistically significant(P<0.05).The positive detection rates of serum PCT,IL-6,CPR and WBC alone and in combination were 96.0%,76.0%,84.0%,93.0%and 100.0%in group A,and 16.0%,22.0%,62.0%,27.0%and 47.0%in group B.The positive detection rate of PCT,WBC,IL-6 and CRP in combination in the two groups was higher than that of serum PCT,IL-6,CPR and WBC alone in this group,and the difference was statistically significant(P<0.05).The positive rates of serum PCT,IL-6,CPR and WBC alone and in combination in group A were higher than those in group B,and the differences were statistically significant(P<0.05).Conclusion PCT,CRP,IL-6,and WBC tests can be applied alone to patients with respiratory tract infections to
