摘  要:剖宫产手术是最常见的手术之一,通常伴有中度至重度疼痛。术后疼痛是产妇的一个普遍担忧。剖宫产术后镇痛的选择多种多样,但是最理想的方法仍没有定论。竖脊肌平面阻滞能提供有效的术后镇痛,这有助于产妇的早期活动、减少阿片类药物的应用、预防术后并发症,逐渐引起临床重视。本文综述近年来竖脊肌阻滞在剖宫产术后镇痛应用的相关研究,包括竖脊肌平面阻滞的作用机制、竖脊肌平面阻滞相关药物的选择及剂量、与其他镇痛方式的对比等。同时添加不同药物作为佐剂的竖脊肌平面阻滞在剖宫产术后镇痛应用的安全性问题,以及对镇痛效果、阻滞时长的影响也是研究热点。竖脊肌平面阻滞在腹部手术术后镇痛中应用广泛,但是其在剖宫产术后镇痛的应用的模式和意义值得进一步研究。Cesarean section is one of the most common surgery and is usually accompanied by moderate to severe pain.Postoperative pain is a concern for puerpera.There are many different types of analgesia after cesarean section,but the optimal method is far from conclusive.Erector spinal plane block can provide effective postoperative analgesia,which can help early maternal activity,reduce the use of opioids,and prevent postoperative complications,and is gradually attracting clinical attention.This study reviews the recent studies on the application of erector spinal plane block in postoperative analgesia after cesarean section,including the mechanism of action of erector spinal plane block,the selection and dosage of drugs related to erector spinal plane block,and the comparison with other analgesia methods.The safety of erector spinal plane block with different drugs as adjuvants in postoperative analgesia after cesarean section,as well as its effect on analgesia and the length of the block are also hot topics of research.Erector spinal plane block is widely used in postoperative analgesia after abdominal surgery,but its application pattern and significance in postoperative analgesia after cesarean section deserve further study.
